初次全髋、膝关节成形术后切口引流期延长影响因素的Logistic分析

Analysis of the Influencing Factors of Extension Drainage in Primary Total Hip and Knee Arthroplasty

目的:调查分析初次全髋、膝关节成形术后切口引流期延长的影响因素,探讨人工关节成形术中科学的引流方法。方法:回顾性分析我院收治的全髋、膝关节成形术患者资料,对术后引流期正常患者和延长患者进行对照研究,采用问卷调查方式收集引流期延长情况资料,对调查因素进行单因素分析,明确单因素对延长引流期是否有统计学意义,将有统计学意义的单因素进行Logistic多因素分析,根据OR值判断更具有独立作用的高危因素。结果:调查因素中4个因素与延长引流期显著相关,引流管发生堵塞(β=1.205,OR=1.794),引流管放置位置(β=1.443,OR=3.136),引流方式(β=1.392,OR=2.793),引流瓶与引流液更换方法是否正确(β=1.417,OR=1.744),是导致引流期延长因素中的高危因素。结论:对导致初次全髋、膝关节成形术后切口引流期延长的影响因素进行调查分析,对于术后促进切口愈合及缩短引流期具有重要指导意义。

Objective：To investigate the influencing factors of extension drainage in primary total hip and knee arthroplasty,to explore the science of drainage methods in primary total hip and knee arthroplasty.Methods：Retrospective analysis of total hip and knee arthroplasty surgery patients with clinical data,with the case-control study,a standardized questionnaire was used for data collection where patients with prolong drainage time in our hospital,the influencing factors were given multi-factor regression analysis in order to find out which factors played independent roles.Results：Four factors was significantly associated with the extention of drainage time in all investigations factors,drainage tube blockage（β=1.205,OR=1.794）,drainage tube in the incision location（β=1.443,OR=3.136）,drainage way （β=1.392,OR=2.793）,replacement method of fluid drainage and drainage bottle（β=1.417,OR=1.744） were significant risk factors. Conclusion：Analysis of the influencing factors of wound drainage time in order to promote wound healing and reduce drainage time.
