进展期胃癌淋巴结转移相关影响因素分析

The Related Factors Analysis on Lymph Node Metastasis of Advanced Gastric Cancer

目的：探讨进展期胃癌的淋巴结转移的相关影响因素。方法：回顾性分析我院自2009年1月-2013年1月间收治的76例进展期胃癌患者临床资料，对患者的淋巴结阳性率和手术清扫的总淋巴结阳性率与临床病理之间的关系进行单因素和多因素分析。结果：76例进展期胃癌患者，淋巴结阳性患者转移率为59.61%（45/76），其中共检出淋巴结869枚，淋巴结的阳性检出率为58.92%（512/869），单因素分析显示淋巴结阳性患者与分化程度、肿瘤的大小、浸润深度、TNM分期和大体分型有关，差异具有统计学意义（P〈0.05）。多因素非条件Logistic回归分析显示分化程度、浸润深度、TNM分期是进展期胃癌淋巴结转移的独立危险因素。结论：进展期胃癌的淋巴结转移率较高，分化程度、浸润深度、TNM分期是进展期胃癌淋巴结转移的独立危险因素。

Objective：To expore the related factors analysis on lymph node metastasis of advanced gastric cancer.Methods：76 patients with advanced gastric cancer were retrospective analyzed from January 2011 to January 2013,the relationship between lymph node positive rate of the patients with surgery,cleaning of the total lymph node positive rate and clinical pathological had a analysis for single factor and multiple factors.Results：76 cases of patients with advanced gastric cancer,the patients with positive lymph node metastasis rate was 59.61% （45/76）,among them 869 lymph nodes were detected,lymph node positive detection rate was 58.92% （512/869）,the single factor analysis showed that both were related to degree of differentiation,tumor size, invasion depth,TNM staging and the general classification.The difference was statistically significant （P〈0.05）.Multi-factor unconditioned Logistic regression analysis revealed that differentiation degree,infiltration depth,TNM staging is independent risk factors for lymph node metastasis of advanced gastric cancer. Conclusion：lymph node metastasis rate of advanced gastric cancer is higher,differentiation degree,infiltration depth,TNM staging is independent risk factors for lymph node metastasis of advanced gastric cancer.
