小汗腺癌

A case of eccrine carcinoma

报告1例头部汗管样小汗腺癌。患者女,73岁。头部斑块20余年,破溃3周。皮肤科检查:头部一3 cm×2 cm浸润性斑块。皮损组织病理检查:真皮中下部及皮下组织见大量向导管分化的基底样细胞嵌于纤维基质中,肿瘤细胞排列成条索状、巢状,伴有大量管腔形成,细胞轻度异形,神经周围见肿瘤细胞浸润。PAS、阿新蓝染色(+)。免疫组化:细胞角蛋白(CK)7、广谱细胞角蛋白(pan-cytokeratin,Pan CK)、上皮膜抗原(EMA)、Ki-67、P63(+);CK20、绒毛蛋白(villin)、甲状腺转录因子1(TTF-1)、巨囊性病液体蛋白-15(GCDFP-15)、S-100蛋白、HMB45(-)。诊断:小汗腺癌。

A case of eccrine carcinoma is reported. A 73-year-old woman presented with more than 20 year- history of a plaque on the head, and 3 week- history of ulcers on the plaque. Physical examination showed a 3 cm×2 cm firm plaque on the head. Histopathology showed the turnout in the dermis and subcutaneous adipose tissue, and was characterized by narrow cords and nests composed of basaloid cells with ductal differentiation. The tumour cells were slightly atypia and extended to the periphery of the nerves. PAS staining and AB staining（＋）. Immunohistochemistry study showed positive staining for CK7, pan CK, EMA, Ki67, P63;negative for CK20, Villin, TTF1, GCDFP15, S-100, HMB45. The diagnosis was confirmed as eccrine carcinoma.
