甲状腺侵袭性纤维瘤病一例报告并文献复习 被引量：1

摘要 1 病例介绍 患者,男性,45岁,主因＂颈部肿物半年＂入院,既往5年前因＂左侧结节性甲状腺肿＂于当地医院行手术治疗（具体手术方式不详）.近半年来无明显诱因出现左侧颈部原手术区肿物,并渐增大,伴吞咽不畅,无声嘶,无发热.查体：颈前可见手术疤痕,气管左侧可触及肿物大约4.0 cm×3.0 cm,质硬,边界不清,可随吞咽上下活动,无压痛,颈部淋巴结未触及.B超示：左侧甲状腺术后,左侧甲状腺区低回声团块,大小4.17 cm×2.53 cm,边界欠清,右侧甲状腺多发小结节,较大的直径0.6 cm,颈部可见淋巴结影,边界清.CT：左侧甲状腺区软组织影,边界不清,请结合临床.入院后给予行左侧甲状腺残余腺体及肿物姑息切除术,术中见肿物与甲状腺后方喉返神经、气管壁及食管粘连明显,应家属要求行姑息性手术.术后病示：左侧甲状腺侵袭性纤维瘤,免疫组化表现为：S-100（-）,NSE（-）,Desmin （＋）,Vimentin（＋）,Actin（-）.术后给予门诊随访.
