2型糖尿病患者伴肾脏损害的肾活检指征分析

The renal biopsy analysis of the patients with type 2 diabetes combined kidney damage

目的探讨2型糖尿病患者伴肾脏损害的肾活检指征。方法对我院2007年1月至2011年6月收治的38例2型糖尿病患者的临床资料进行回顾性分析,并对所有患者进行肾活检、眼底和常规实验室检查。结果糖尿病肾病(DN)20例(占52.63%);非糖尿病肾损害(NDRD)8例(占21.05%)。伴有糖尿病眼部病变(糖尿病视网膜病、晶状体或玻璃体浑浊、白内障)者10例,肾活检均为DN。结论肾活检是确诊DN或NDRD的金指标,且糖尿病眼部病变预测糖尿病肾病的特异性为100%。糖尿病合并肾损害患者应尽早肾活检,以明确诊断并给予正确的治疗。

Objective：To investigate indications for renal biopsy of the the type 2 diabetic patients with kidney damage.Methods：The retrospective analisis about 47 cases with type 2 diabetes clinical data from January 2010 to June 2011 in our hospi- tal, and all patients were tested about renal biopsy, eye and conventional laboratory tests.Results：The 28 cases of diabetic nephropathy （DN） were 59.57% of all patients;and the 11 cases of non-diabetic kidney damage （NDRD） were 23.40%. The renal biopsy of 14 patients associated with diabetic eye disease （diabetic retinopathy, lens or vitreous opacity, cataract） were DN. Conclusion：The diagnosis of DN or NDRD biopsy is the gold standard, and the specificity about diabetic eye disease diabetic redicted nephropathy was 100%. Diabetic patients with renal biopsy as soon as possible to give the correct diagnosis and treatment.
