摘要
目的应用超声二维应变显像技术对酒精性心肌病患者的左心室旋转及扭转运动影响进行评价。方法A组(对照组)健康志愿者55例。B组(酒精性心肌病组)39例,每日啤酒≥4瓶或饮白酒≥150g,每周6~7d,10年以上,饮酒量符合酒精性心肌病诊断标准。对两组进行常规超声及2DSI参数测定。结果与A组比较,B组常规超声参数(LVDd、LVDs、IVSTd、IWTd、LVEF和E/A,P均〈0.01)明显减低,左心室短轴二尖瓣水平、心尖水平的心肌收缩期旋转角度、左心室收缩期扭转角度峰值应变及舒张功能参数E/A值明显减低,差异均有显著统计学意义(P〈0.01)。结论2DSI技术可对酒精性心肌病患者的左心室心肌旋转角度及左心室扭转运动进行准确评估。
Objective To evaluate the effect of left ventricular rotation and twist of patients with alcoholic cardiomyopathy by two-dimensional strain imaging(2DSI). Methods 94 chronic alcohol abusers were divided into two groups:group A (the control group, n=55) and group B(more than 5 bottles of beer or more than 170 g ethanol daily for 12 years or more, 5 to 6 days per week, n=39). The parameters of the conventional echocardiography and 2DSI were obtained respectively. Results Compared with group A, the conventional echocardiographic parameters in group B decreased significantly (P〈0.01). The leR ventticular short-axis views at the levels of mitral annulus, apex peak systolic rotation two-dimensional strain of left ventricular short-axis, the peaks systolic torsion of left ventricular and diastolic function parameters E/A in group B lowered down remarkably (P〈 0.01). Conclusions 2DSI can effectively assess the change of left ventricular peak systolic rotation and torsion of alcohol-induced myocardial damage. Conclusion 2DSI can effectively assess the change of left ventricular peak systolic rotation and torsion of patients with alcoholic cardiomyopathy,
