Electroencephalogram nonlinear analysis in mild cognition impairment patients

摘要 目的从非线性动力学分析的角度探讨轻度认知功能障碍(MCI)患者脑电变化的特征。方法选取MCI患者19例,其中非痴呆型血管性认知功能障碍(VCIND)患者10例,遗忘型轻度认知功能障碍(aMCI)患者9例。MCI患者经中文版简易智能状态检查(MMSE)、蒙特利尔认知评估量表(MoCA)、总体衰退量表(GDS)、临床痴呆评定量表(CDR)、Hachinski缺血指数(HIS)、汉密尔顿抑郁量表(HDRS)、日常生活活动能力量表(ADL)评定,在安静状态下采集脑电信号后行非线性分析并与10位年龄匹配的健康老年人(HE)行组间比较;同时,VCIND亚组与aMCI亚组行MCI组内比较。结果 MCI组与HE组比较,脑电非线性参数关联维数(D2)、点关联维数(PD2)、复杂度(Cx)平均值减低且差异均有统计学意义(P均<0.05);aMCI亚组与VCIND亚组比较,脑电非线性参数Cx平均值增高,李亚普诺夫指数(LE)平均值减低且差异均有统计学意义(P均<0.05)。结论脑电非线性分析能够较早客观判断脑功能的改变,对MCI的诊断及亚型的判定有一定的辅助价值。 Objective To investigate the changes of the EEG in mild cognition impairment (MCI) patients by means of nonlinear analysis. Methods A total of 19 patients with MCI were enrolled in this study which included 10 vascular cognitive impairment not dementia (VCIND) and 9 amnestic mild cognition impairment (aMCI) patients from January 2012 to March 2013. Ten healthy elder persons (HE) were selected as control. Mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA), global deterioration acale (GDS), clinical dementia rating scale (CDR), Hachinski ischemic index (HIS), Hamilton depression rating scale (HDRS), activity of daily living (ADL) were completed and EEG were recorded for nonlinear analysis. The EEG nonlinear parameters were compared among groups (MCI versus HE and aMCI versus VCIND ). Results The mean values of correlation dimension (D2), point wise correlation dimension (PD2) and complexity (Cx) were signifcantly lower (P〈O.05) in MCI group than those in HE group; As compared with VCIND subgroup, the mean value of Cx was significantly increased and the mean value of Lyapunov exponent (LE) was significantly lowered in aMCI subgroup (P〈0.05). Conclusion EEG nonlinear analysis might objectively be used to evaluate the brain functional changes and play a role in MCI diagnosis and its subgroup determination.
