摘要
目的联合检测胃癌患者D-D、CRP和肿瘤标志物,探讨其对胃癌早期诊断的价值。方法电化学发光法测CA72-4、CEA、CA19-9、CYFRA21-1、CA125;免疫比浊法测D-D、CRP,并比较胃癌组、良性胃病组、健康对照组间各指标的阳性率及7项指标单一或联检的敏感度和特异度。结果胃癌组7项指标显著高于其他两组(P<0.01)。良性胃病组D-D、CRP、CA72-4、CEA、CA19-9水平高于健康对照组(P<0.05);良性胃病组及健康对照组CYFRA21-1、CA125水平比较,差异无统计学意义(P>0.05)。胃癌组患者CRP、D-D、CA72-4、CEA、CA19-9、CYFRA21-1、CA125单项阳性检出率分别71.21%、62.12%、74.24%、54.55%、59.09%、31.82%、27.27%,均明显高于其他两组(P<0.01或P<0.05)。结论联检D-D、CRP、CA72-4、CEA、CA19-9、CYFRA21-1、CA125可提高胃癌诊断的敏感度,有助于胃癌的早期诊断、治疗监测及预后判断。
Objective To investigate Methods E1 the joint diagnostic value of D - D, CRP and tumor markers in early gastric cancer. technology was used to detect the levels of carbohydrate antigen 72 - 4 ( CA72 - 4), carcinoembryonic antigen ( CEA), carbohydrate antigen 19 -9 ( CA19 -9), cytokeratin 19 ( CYFRA21 - 1 ) and carbohydrate antigen 125 ( CA125), while the turbidimetric immunoassay was used to detect the D - D and CRP. Then the positive rates of these indxes among gastric cancer group, benign gastric disease group and healthy control group, as well as the sensitivity and specificity of single index detection and combined detection were compared. Results The levels of the above seven indexes in gastric cancer group were significantly higher than those in the other two groups(P 〈 0.01 ). The levels of DD, CRP, CA72 - 4, CEA, CA19 - 9 were higher in benign gastric disease group than those in healthy control group(P 〈 0.05), while the levels of CYFRA21 - 1 and CA125 had no statistical difference between the two groups (P 〉 0.05 ). The positive rates of CRP, D - D, CA72-4, CEA, CA19-9, CYFRA21 -1, CA125 were 71.21%, 62. 12%, 74.24%, 54.55%, 59.09%, 31.82%, 27.27% respectively in gastric cancer patients, all were significantly higher than the other two groups (P 〈 0.01 or P 〈 0.05 ). Conclusion Combined detection of D- D, CRP, CA72-4, CEA, CA19-9, CYFRA21- 1, CA125 can improve the diagnostic sensitivity of gastric cancer, it is beneficial for early diagnosis, treatment monitoring and prognosis evaluation of gastric cancer.
