整体护理在颈动脉狭窄患者行颈动脉内膜剥脱术中的应用

Application of holistic nursing in carotid endarterectomy for patients with carotid stenosis

目的：探讨整体护理在颈动脉狭窄患者行颈动脉内膜剥脱术中的应用价值。方法回顾性分析300例颈动脉狭窄行颈动脉内膜剥脱术患者的临床资料，将其随机分为观察组和对照组各150例，对照组患者给予常规护理，观察组在对照组的基础上给予整体护理，对比分析两组患者焦虑、抑郁自评情况、并发症发生情况及对护理工作满意度。结果术后1周观察组焦虑、抑郁量表评分分别为（33．0±3．5），（32．3±3．1）分，明显优于对照组的（44．2±4．1），（43．4±4．7）分，两组比较差异有统计学意义（ t值分别为13．937，13．225；P＜0．05）；观察组并发症总发生率为4．7％（7/150），对照组并发症总发生率11．3％（17/150），观察组并发症发生率低于对照组，两组比较差异有统计学意义（χ2＝4．529，P＝0．033）；观察组患者对护理工作满意度明显优于对照组，差异有统计学意义（Z＝-2．784，P＜0．05）。结论将整体护理应用于颈动脉狭窄行颈动脉内膜剥脱术患者，可改善患者的焦虑和抑郁状态，降低术后并发症的发生率，提高患者对医院的满意度。

Objective To explore the application of holistic nursing in patients with carotid artery stenosis of carotid endarterectomy .Methods Totals of 300 cases of patients with carotid artery stenosis of carotid endarterectomy were divided into control group （ n =150 ） and observation group （ n =150 ） , control group was cared with conventional care , while observation group was cared with holistic nursing .Self-rating anxiety and depression , complications , and satisfaction of nursing job were compared .Results Depression and anxiety surveys showed that：one week after surgery ,scores of anxiety and depression in observation group were （33.0 ±3.5） and （32.3 ±3.1）, while those in control group were （44.2 ±4.1） and （43.4 ±4.7） respectively, and the differences between two groups were statistically significant （ t =13.937, 13.225, respectively;P＆lt;0.05）.In observation group, the total incidence of complications was 4.7%（7/150）,and that in control group was 11.3%（17/150）,with a significant difference between the two groups （χ2 =4.529,P=0.033）.Satisfaction survey showed that nursing job satisfaction of the observation group was significantly better than that of control group （Z =-2.784,P ＆lt;0.05）.Conclusions Holistic nursing can improve the depression and anxiety of patients , decrease the incidence of complications , and improve patients ’ satisfaction in carotid endarterectomy for patients with carotid stenosis .
