退变性腰椎滑脱的手术治疗 被引量:7

摘要 目的探讨后路椎管减压、钉棒系统复位内固定、斜向单枚融合器植入椎间融合+椎板关节突植骨融合治疗退变性腰椎滑脱症的疗效。方法对42例退变性腰椎滑脱患者进行后路椎管减压、钉棒系统复位内固定、斜向单枚融合器植入椎间融合+椎板关节突植骨融合手术治疗。其中I度滑脱36例,Ⅱ度滑脱6例。31例行椎板开窗减压短钉棒系统固定、椎板关节突植骨+斜向单枚融合器植入椎间融合术,11例行全椎板减压短钉棒系统固定、椎板关节突植骨+斜向单枚融合器植入椎间融合术;3例双椎体滑脱采用全椎板减压、钉棒系统固定、椎板关节突植骨+斜向单枚融合器植入椎间融合术,2个滑脱椎体间隙均通过椎间融合器进行椎体间融合;39例单椎滑脱行双椎体钉棒系统固定,3例双椎体滑脱行三椎体钉棒系统固定。结果42例患者随访12~36个月,平均20个月。术后1周复查x线片显示内固定器械位置良好,40例完全复位,2例仍有I度滑脱,其中1例术前为Ⅱ度滑脱,1例术前为双椎滑脱,复位率95.2%。36例术后2周内神经受压症状消失,6例术后出现神经根刺激症状加重,5例经对症治疗后于3个月内缓解,1例症状未缓解者于术后8个月将内固定钉棒取出后症状缓解。不全瘫者1例,经甲强龙500mg,1天1次冲击,3d好转,3周恢复。所有患者均在术后12—18个月获得骨性融合,其中25例在术后1年后即融合。临床疗效评分:优27例,良12例,一般3例,优良率为92.9%。结论后路椎板减压、钉棒系统固定、椎板关节突植骨+斜向单枚融合器植入椎间融合术治疗退变性腰椎滑脱的复位作用满意,临床疗效良好;术前应全面评估其椎管狭窄及神经根受压情况,准确判断症状的原因,来源部位和范围,再结合相关的影像学检查制定出一个合理的手术方案;术中在减压、复位、固定、融合等方面有所侧重,才能取得较好的疗效。
