超声心动图诊断单侧肺动脉缺如的规律性研究

STUDY OF ECHOCARDIOGRAPHY IN DIAGNOSIS OF UNILATERAL ABSENCE OF PULMONARY ARTERY

目的探讨超声心动图诊断单侧肺动脉缺如的特征及规律,提高诊断率。方法回顾性分析经心脏外科手术或心血管造影检查证实的17例单侧肺动脉缺如的超声心动图资料。结果17例肺动脉缺如中左肺动脉缺如占88.24%(15/17),右肺动脉缺如占11.76%(2/17)。伴有其他心脏畸形占82.35%(14/17),其中法洛四联症合并左肺动脉缺如多见占35.29%(6/17)。17例单侧肺动脉缺如超声诊断14例,诊断率82.35%。超声心动图表现为正常的主肺动脉分叉结构消失,缺如一侧的肺由主动脉弓或降主动脉通过发出的侧支循环供血。结论超声心动图对诊断一侧肺动脉缺如具有较高的准确性,可为临床提供重要的参考信息。

Objective To summarize the echocardiography features in the diagnosis of unilateral pulmonary artery absence which could help improve the ultrasound diagnosis rate.Methods Seventeen cases of patients with one side of pulmonary artery absence were enrolled in this study,the outcomes were confirmed by analysis in surgery,angiography or multi-slice CT examination.Results In the 17 cases of pulmonary artery absence,the left pulmonary artery agenesis accounted for 88.24%（15/17）,and the right pulmonary artery agenesis accounted for 11.76%（6/17）.Echocardiography showed normal main pulmonary artery bifurcation structure disappeared.Absence of the pulmonary artery side collateral circulation usually came from aortic arch or descending aorta.The unilateral pulmonary artery absece accompanied other heart defects accounted for 82.35%,mostly was tertralogy of Fallot with left pulmonary artery absence for 35.29%（61/17）.In this 17 cases of unilateral pulmonary artery absence,ultrosund diagnosis was 14 cases and the diagnosis rate was 82.35%（14/17）.Conclusion Echocardiography is non-invasive effective in diagnozing one side of pulmonary artery absence,it could provide important reference information for clinic.
