摘要
为通过观察结直肠癌患者手术前后肠道中双歧杆菌(Bifidobacteriumspp.)、乳酸杆菌(Lactobacillus)、粪肠球菌(Enterococcus如ecalis)和拟杆菌(Bacteroidaceae)的变化情况,分析肠道菌群的改变在结直肠癌发病及治疗中的作用及意义,收集正常健康人粪便标本60份、结直肠癌患者术前及术后粪便标本各60份,应用平板活菌计数法和荧光定量聚合酶链反应(PCR)法检测粪便标本中各种菌群的数量,同时采集患者的外周血标本,应用显色法检测D-乳糖和内毒素的含量。结果显示,结直肠癌患者术前粪便中乳酸杆菌、双歧杆菌数量均比正常健康人少,P〈O.05或P〈O.01;粪肠球菌、拟杆菌数量均比正常健康人多,P〈O.05或P〈O.01。结肠癌患者术后粪便中乳酸杆菌、双歧杆菌数量进一步减少,P〈0.05或P〈0.01;粪肠球菌、拟杆菌数量进一步增多,P〈O.05或P〈O.01。结直肠癌患者血清I)I乳糖、内毒素含量比正常健康人高,P〈O.Ol,手术后其含量进一步升高,P〈O.Ol。结果表明,肠道菌群的变化与结直肠癌的发生、发展及预后有一定关系。
Through observation on the change status of the enteric bacterium flora such as Bifidobacteriurn spp. , Lactobacillus , Enterococcus faecalis and Bacteroidaceae this study was to analyzed the action and significance of enteric bacterium flora change in the pathogenesis and treatment of co|orectal carcinoma. Authors collected 60 feces specimen from health person, 60 preoperative feces specimen and 60 postopera= tive feces specimen from colorectal carcinoma patients, by using plate-mehtod for counting viable bacterium and fluorescent quantitative polymerase chain reaction(PCR) detected the contents of each flora of those specimen;at this time, collected the peripheral blood specimen of patients, by using chromogenie test detec- ted the contents of D-lactose and endotoxin. As results, the contents of Lactobacillus and Bifidobacterium sppl from fecel specimen of patients before surgery were lower than that of health person( P 〈0.05 or P 〈0.01) ,also further decreased after operation( P 〈0.05 or P 〈0.01), but the contents of Enterococcus faecalis and Bateroidaceae higher( P 〈0. 05 or P 〈0.01) ~also further increased( P 〈0. 05 or P 〈0. 01) ; the contents of peripheral serum D-lactose and endotoxin in patients were higher than that in health person ( P 〈0.01) ,also further increased after operation( P 〈0.01). Results show that the change of enteric bac- terium flora is related tO the pathogenesis,develol0rnent and prognosis of colorectM cancer certainly.
