TLIF技术在腰椎间盘翻修手术中应用的选择和疗效分析 被引量:2

摘要 目的探讨经椎间孔腰椎椎体间融合术(Transforaminal Lumbar Interbody Fusion,TLIF)在腰椎翻修手术中的应用和临床疗效。方法 2008-06-2012-08应用TLIF技术治疗28例FBSS患者。采用日本骨科学会评分系统(Japanese Orthopaedic Association,JOA)评价治疗效果,Suk方法评价植骨融合情况。结果 2例术中出现硬脊膜损伤,1例术后出现脑脊液漏,1例术后出现切口内血肿,无永久性神经损伤者。术后JOA评分,优19例,良5例,可4例,改善率85.7%,术后12个月根据Suk标准椎间植骨均获得骨性融合。结论 TLIF术式具有并发症少、神经根管减压彻底、能够重建脊柱的稳定性、恢复椎间高度及生理前凸等优点,可以获得满意的临床效果。 Objective To explore the application and clinical effect of transforaminal lumbar in- terbody fusion (TLIF)teehnique in lumbar revision surgery. Methods From June 2008 to August 2012, 28 patients(male/female: 18/10,average age: 50.2 years old)were diagnosed failed back surgery syndrome (FBSS)and received TLIF surgery. Japanese Orthopaedic Association (JOA) score system was adopted to evaluate the clinical results. Results 2 eases had dural rupture intraoperation, 1 ease had eerebrospinal fluid leakage after operation, and 1 ease occurred ineisional hematoma after operation. No permanent neurological complication was observed. All eases were followed up for 12-52 months (average 24.7 months). Based on the JOA score system for efficacy evaluation,19 eases were excellent,5 eases were good and 4 eases were fair. The clinical improvement rate was 86.5%. Excellent bone grafts fusion was observed in all patients 12 months after operation. Con- clusion TLIF has the advantages of less complication,satisfying effects of decompression, recon- structing of spinal stability, remodeling the height of vertebral body and physiological anteeurva- ture,TLIF demonstrated as a superior choice for lumbar revision surgery.
