广州地区202例住院患者维生素D水平现状分析 被引量:4

摘要 目的分析广州地区住院患者的维生素D水平的现状与原因。方法在广州地区202例年龄在>40岁的住院患者中,检测血清25羟维生素D(25(OH)D)、甲状旁腺激素(PTH)、骨钙素(OCN)水平。所有研究对象均用双能X线吸收仪检测腰椎和股骨近端骨密度T值,同时记录患者的用药情况。结果 202例住院患者25(OH)D平均水平为(19.28±6.25)ng/ml。25(OH)D水平与PTH、OCN呈负相关,而腰椎T值、股骨T值与25(OH)D水平呈正相关(p<0.05)。VitD治疗组与未治疗组间的25(OH)D水平比较无显著差异。在202例患者中,VitD正常者仅占3.5%,VitD不足者约有96%,其中不足者占40.6%,缺乏者占49.5%,严重不足者占6.4%。结论住院患者维生素D水平普遍低下,绝大部分患者存在VitD不足,需要重视及积极治疗。在骨质疏松症患者中,补充足量维生素D,有利于增加骨密度,维持骨骼健康,注意监测25(OH)D水平。 Objective To analysis vitamin D level situation in hospitalized patients in Guangzhou. Methods 202 old hospitalized patients age 〉 40years were detected of serum 25 hydroxy vitamin D ( 25 ( OH ) D ) , parathyroid hormone (PTH) , osteocalcin (OCN) . All of the subjects were detected of lumbar spine and proximal femur bone mineral density T value by dual - energy X - ray absorptiometry, at the same time patients' medication was documented. Results 1, In 202 inpatients the average level of 25 (OH) D was ( 19.28 ± 6. 25) ng/ml. From 40 to 70 years old, 25 (OH) D levels were gradually re- duced by year, 60 - 69 group minimum, but 70 - 79 group increased, 80 - 90 group was lowest. 2, 25 (OH) D levels and PTH, OCN was negatively related, and lumbar T, femoral T values and 25 (OH) D levels were positively correlated. 3, The difference of 25 (OH) D level between VitD treatment group and untreated group was not significant. 4, In 202 inpatients, the normal VitD status accounted for only 3.5% , VitD deficiency about 96% , including insufficiency of vitamin D for 40. 6% , VitD deficiency for 49.5% , serious VitD deficiency for 6.4%. Conclusion Inpatients vitamin D level is generally low, most pa- tients had VitD deficiency, so these patients need to pay more attention and actively treatment, in patients with osteoporosis, vitamin D supplement is condutive to the increase of bone density and the maintenance of bone health.
