摘要
目的 探讨原发性高血压病患者静息心率与大动脉脉搏波速度(pulse wave velocity,PWV)的相关性.方法 应用脉搏波速度自动测量系统测量颈动脉-股动脉脉搏波速度(carotid-femoral PWV,CFPWV)作为反映主动脉僵硬度的指标,对644例原发性高血压患者(男340例,女304例,年龄18~82岁,平均52±13岁)进行脉搏波速度检测,并检测空腹血糖、血脂、肾功能、尿酸等血生化指标.结果 CFPWV在心率(HR)<60次/min、60≤HR<70次/min、70≤HR<80次/min和HR≥80次/min 4组间逐渐升高(P<0.01),相关性分析显示静息心率与CFPWV呈显著正相关(P<0.001),多元逐步回归分析得出年龄、收缩压、HR、尿酸及血糖是影响CFPWV增加的独立因素.结论 静息心率增快与高血压患者主动脉脉搏波速度增快相平行,可能参与大动脉僵硬度增加的病理生理过程.
Objective To study the relationship between resting heart rate(HR) and large arterial stiffness in patients with essential hypertension. Methods Automatic pulse wave velocity(PWV) measuring system was applied to examine carotid-femoral PWV(CFPWV) as the parameter reflecting aortic arterial stiffness. Six hundred and forty-four hypertensive patients aged 18-82 years(340 males and 304 females, mean age 52 ± 13 years) were recruited. Results CFPWV increased gradually with increasing heart rate( P 〈0.01 ). HR and CFPWV were positively and significantly correlated( P 〈0. 001). A stepwise multiple regression analysis demonstrated that age, systolic blood pressure, heart rate,uric acid and fasting glucose were positively and independently related to CFPWV. Conclusion Increased resting heart rate occurs in parallel with the elevated automatic PWV, which might involved in the pathophysiology of vascular remodeling in patients with essential hypertension.
