不同镇痛方法对剖宫产患者血清白细胞介素-6、10影响的比较 被引量：12

摘要 目的比较3种不同镇痛方法对剖宫产患者血清白细胞介素(IL)-6、IL-10的影响。方法对30例足月剖宫产产妇根据需要分别肌肉注射哌替啶100 mg(NC组)、5 mg吗啡+150 mg进行罗哌卡因硬膜外持续镇痛(MR组)、浓度为1.5μg/mL的舒芬太尼+氟哌利多5 mg进行静脉持续镇痛(SF组),每组10例,采用电子镇痛泵,总容量为100 mL,记录术后4、12、24、48 h的VAS疼痛评分,并抽取外周静脉血,检测血清IL-6、IL-10的浓度,统计不良反应的发生情况。结果 MR组和SF组镇痛效果良好,两组12、24 h VAS评分明显低于NC组,差异有统计学意义(P<0.05);3组术后IL-6、IL-10均有不同程度升高,MR组和SF组各时间点差异无统计学意义(P>0.05),但在12、24 h两组与NC组差异有统计学意义(P<0.05)。结论静脉和硬膜外两种镇痛方法对剖宫产患者均能取得良好的镇痛效果,且能降低术后血清IL-6的水平,增加抗炎因子IL-10的释放,有效地维持术后炎性因子的平衡,减轻术后的炎症反应,但吗啡硬膜外镇痛不良反应明显多于舒芬太尼静脉镇痛。
