Exploration on TCM Treating Rules of Chronic Diarrhea by Using Medical Case Statistics

目的:探究久泻的中医药治疗规律。方法:从《中国百年百名中医临床家》系列丛书中108本分册中筛选出符合标准的68则验案,记录每则医案的处方用药、治则、治法,然后再分别对所使用药物的性味、归经等方面进行相关的统计学分析。结果:治疗久泻的治则可概括为运脾除湿、温肾涩肠、柔肝疏肝三法,可以具体划分为健脾、除湿、固涩、疏利、温肾、祛风、柔肝、消导、升阳九种治疗方法。其中健脾除湿法最为重要,贯穿各种久泻的治疗中。使用最多的药物处方是参苓白术散和平胃散加减,药物多数归脾、胃、肺、大肠经;药性多性平或温热;药味多辛、苦、甘。结论:久泻的治疗应以健脾除湿为基本大法,同时兼顾肝肾。

Objective：To explore the TCM treating rules of chronic diarrhea. Methods： By seeking out 68 proved cases from 108 divisions of the series named Hundred TCM Clinical Doctors o^Hundred Years in China, by recording the prescription, drugs, treatment rules, therapeutic methods in each case ; and then we used the statistics methods to analyse the nature and flaour, meridial distribution and other aspects of the Chinese herbal medicines which were concerned. Resuhs ： The common treatment rules of chronic diarrhea can be summarized into three kinds ： operating the spleen and eliminating the moisture ; warming the kidney and astricting the intestine ; softening the liver and soothing the liver. In detail, the treatment can be divived into nine therapeutic methods, namely： operating the spleen; eliminating the moisture ; astricting the intestine ; softening the liver; warming the kidney ; dispelling the wind ; softening the liver; relieving dispersion ; promoting the yang. Among them, operating the spleen and eliminating are the most important and through out all the treating rules of chronic diarrhea. The most widely-used Chinese medicine prescriptions are the addition and subtraction of Senling Baizhn Powder and Pinwei Powder. The Chinese medicines mostly belong to collateral channels of spleen, stomach, lung, large intestine. Their natures mostly are mild , warm and heat. Their flavours mainly are symplectic, bitter and sweet. Conclusions ： TCM treating rules of chronic diarrhea should be based on the treatment rule of operating the spleen and eliminating the moisture. Meantime we can not ignore the liver and kidney.
