妊娠期糖尿病的临床规范治疗体会

Experience of clinical standard treatment in gestational diabetes mellitus

目的探讨妊娠期糖尿病的临床规范治疗方法。方法选择本院2010年1月。2013年6月收治的150例妊娠期糖尿病患者的临床资料进行回顾性分析，将其分为观察组和对照组，对照组75例患者孕36周后入院确诊为妊娠糖尿病，无正规产前检查记录，观察组75例自妊娠开始在本院进行产检监测，孕24，28周发现糖尿病，并进行规范治疗，观察妊娠期糖尿病对孕妇及胎儿的影响。结果观察组高血压、产后出血、羊水过多、肩难产等并发症发生率低于对照组（P〈O．05），观察组流产和早产、胎儿生长受限、胎儿窘迫、新生儿窒息、巨大儿等并发症发生率均低于对照组（P〈O．05）。结论妊娠期糖尿病对母婴有明显不良影响，临床应加强妊娠期糖尿病筛查．尽早诊断，定期产检，采用规范治疗可改善母婴结局，降低并发症发生率。

Objective To explore the clinical standard treatment of estational diabetes mellitus.Methods Clinical data of 150 cases of patients with gestational diabetes mellitus in our hospital from January 2010 to June 2013 were ana- lyzed retrospectively,the patients were divided into 2 groups,75 cases of control group admitted to our hospital after 36 weeks gestation,whout regular antenatal examination records,and were diagnosed with gestational diabetes,75 cases of observation group were given prenatal monitoring in our hospital since pregnancy,and were found with diabetes at 24 to 28 weeks gestation,then they were given with clinical standard treatment,the influence of gestational diabetes for preg- nant women and fetus Were observed.Results The complication rate of hypertension,postpartum hemorrhage,polyhydram- nios and shoulder dystocia in observation group was lower than that in the control group （P〈0.05）; the complication rate of abortion and premature delivery,fetal growth restriction,fetal distress,neonatal asphyxia and fetal macrosomia in ob- servation group was lower than that in the control group （P〈0.05）.Conelusion Gestational diabetes has obvious adverse effects on pregnant women and infant,clinic should strengthen gestational diabetes screening,make early diagnosis and regular production inspection,and given standard treatment inorder to improve outcomes of pregnant women and infant.
