膝关节髌骨软骨症关节镜治疗的疗效对比研究

Comparative Study on the Curative Effect of Knee Arthroscopic Treatment of Patellar Cartilage Disease

目的：探究膝关节髌骨软骨症关节镜治疗的疗效。方法：对我院2008年8月～2010年7月收治的53例膝关节髌骨软骨症患者进行回顾性分析，其中29例患者采用关节镜治疗，纳入观察组，其余24例患者采用关节内注射透明质酸钠治疗，纳入对照组，观察两组患者治疗前后膝关节评分（HSS）及治疗效果。结果：两组患者治疗前平均HSS评分分别为21.2及20.9分，治疗后分别升至46.7及35.4分，其HSS评分均在治疗后有所上升，且观察组上升程度明显高于对照组，数据对比存在显著统计学差异；观察组，优12例，良13例，优良率86.2%，对照组，优6例，良9例，优良率62.5%，观察组疗效明显优于对照组，数据对比存在显著统计学差异（P〈0.05）。结论：关节镜治疗膝关节髌骨软骨症较常用透明质酸钠注射治疗具有膝关节功能改善程度好、恢复快、安全性高等多种优势，且治疗成本低，患者痛苦小，值得临床广泛推广。

Objective：To explore the curative effect of knee arthroscopic treatment of patellar cartilage disease. Methods：In our hospital in 2008 August-2010 year in July 53 cases of knee joint patellar cartilage disease were analyzed retrospectively, in which 29 cases were treated with arthroscopic treatment, in the observation group, the other 24 patients were treated with intra-articular injection of sodium hyaluronate injection therapy, were included in the control group, to observe the knee score before and after treatment in the two groups （ HSS ） and the effect of treatment. Results：Two groups of patients before treatment, the average HSS scores were 21.2 and 20.9, respectively, to 46.7 and 35.4 after treatment, the HSS scores have increased after treatment, and the observation group increased significantly higher than that of the control group, there was significant difference in comparison of the data;the observation group 12 cases of excellent, good in 13 cases, excellent rate of 86.2%, control group 6 cases of excellent, good in 9 cases, the excellent and good rate was 62.5%, the observation group was better than control group, there was significant difference in comparison of the data （P〈0.05）. Conclusion： Treatment with knee joint function improvement degree good, fast recovery, high security advantages of arthroscopic treatment of patellar cartilage disease more commonly used sodium hyaluronate injection, and the treatment of low cost, little pain of patients, is worthy of clinical application.
