摘要
目的:分析视黄醇结合蛋白(RetinolBindingProteinRBP)水平在糖尿病肾病(Diabeticnephropathv,DN)悍期肾损伤诊断中的应用价值。方法:在日立7180全自动生化分析仪上检测87例(分为单纯DM组、早期DN组、临屎DN组)DN患者(实验组)及80例健康者(对照组)血清RBP(SRBP)和尿液RBP(URBP)水平,同时对常规检测指标血清尿微量白蛋白(mAlb)、尿素(UREA)和肌酐(CREA)的水平检测,并对其检测结果进行比较统计学分析。结果:实验组SR_BP、URBP、mAlb、UREA和CREA水平均高于对照组(P〈0.05或P〈0.01):单纯DN组:SRBP和URBP高于对照组(p〈0.05),mAlb、UREA和CREA高于对照组(P〉0.05);早期DN组:SRBP和URBP明显高于对照组(p〈0.01),mAlb高于对照组(P〈0.05),UREA和CREA高于对照组(P〉0.05):临床期DN组:SRBP、URBP和mAJb显著高于对照组(p〈0.01),UREA和CREA高于对照组(P〈0.05)。结论:RBP检测可作为DN早期肾损伤的敏感指标,对DN患者进行RBP水平的监测,比监测mAlb、UREA和CREA水平能更早地反映肾小管损伤情况,以达到早期诊断的目的。
Objective : To analysis the of retinol binding protein (Retinol Binding Protein RBP) level in patients with diabetic nephropathy (Diabetic nephropathy, DN)) for the value of early diagnosis of kidney injury. Methods : In the Hitachi 7180 automatic biochemical analyzer (divided into DM group, DN group, DN group, early clinical) in patients with DN (experimental group) 87 cases were detected and 80 healthy subjects (control group) and serum KBP (SKBP) and urine RBP (UKBP) level, while the conventional detection of serum indexes ofmicroalbuminuria (mAlb), urea (UREA) and creatinine (CKEA) levels were detected, and the detection results were compared statistically analysis.Results : The experimental group SRBP, URBP, mAlb, UREA and CREA levels were higher than those in the control group (P〈0.05 or P〈0.01); DN group: SRBP and URBP higher than that of the control group (p〈0.05), mAlb, UREA and CKEA higher than that of the control group (P 〉 0.05); the early DN group: SRBP and URBP were significantly higher than those in the control group (p〈0.01), mAlb higher than that of the control group (P〈0.05), UREA and CKEA higher than that of the control group (P 〉 0.05); clinical stage DN group: SRBP, URBP and mAlb were significantly higher than those in the control group (p〈0.01), UREA and CREA higher than that of the control group (P〈0.05). Conclusions : RBP testing can be used as a sensitive index of early renal damage in DN, monitoring of RBP levels with DN patients than hematuria, monitoring of mAlb, UREA and CREA levels can reflect the condition of earlier renal tubular injury, in order to achieve the goal of early diagnosis.
