腹腔镜与开腹手术治疗良性卵巢囊肿的临床效果比较

Comparison of effect between laparoscopy and laparotomy for benign ovarian cysts

目的比较腹腔镜与开腹手术治疗良性卵巢囊肿的临床效果。方法选择2010年1月至2013年1月在我院住院治疗的良性卵巢囊肿患者60例,其中行腹腔镜手术治疗的30例卵巢囊肿患者设为观察组,其余30例设为对照组,对比观察两组患者的手术时间、术中出血量、术后肛门排气时间、住院时间及进行病理分类,观察比较囊肿的类型:卵巢单纯性囊肿、卵巢冠囊肿、巧克力囊肿、畸胎瘤。并观察记录术后发生皮下气肿、气体栓塞、大出血、脏器损伤和感染、大出血、发热、腹腔脏器粘连、腹痛等并发症情况。结果腹腔镜组的手术时间(68.1±3.7)min,明显短于对照组,腹腔镜组术中出血量(63.8±5.7)ml,明显少于对照组,观察组患者术后肛门排气时间(14.67±3.78)h,明显快于对照组,观察组患者术后住院时间(5.25±1.28)d,明显短于对照组。观察组患者无1例发生皮下气肿、气体栓塞、大出血、脏器损伤和感染等手术并发症。对照组出现发热4例,大出血3例,腹腔粘连2例,腹痛7例,感染2例,对照组术后并发症明显多于观察组(P<0.05)。结论与开腹手术治疗良性卵巢囊肿的临床效果比较,腹腔镜手术具有创伤小、术中出血少、术后恢复快、并发症少等优点,值得广泛推广和应用。

Objective To compare the laparoscopic and open surgery for benign ovarian cysts clinical results.Methods January 2010 -January 2013 in our hospital treated 60 patients with benign ovarian cysts , including laparoscopic surgery , 30 patients with ovarian cysts to the observation group , and the remaining 30 cases to the control group, contrast observed in patients with operative time , blood loss, postoperative anal exhaust time , hospitalization time and pathological classification , observation and comparison cyst types： simple ovarian cysts, ovarian cysts crown, chocolate cysts, teratomas.And observation of postoperative subcutaneous emphysema, air embolism, bleeding, organ damage and infection, bleeding, fever, abdominal adhesions, abdominal pain and other complications .Results Laparoscopic surgery time 68.1 ±177;3.7min, significantly shorter than the control group , the laparoscopic group blood loss 63.8 ±177;5.7ml, significantly less than the control group , the observation group patients flatus 14.67 ±177;3.78h, significantly faster than the control group , the observation group postoperative hospital stay 5.25 ±177;1.28d, significantly shorter than the control group .The patients no case of subcutaneous emphysema , air embolism, bleeding, organ damage and infection and other surgical complications.Control group, four cases of fever, bleeding in 3 cases, 2 cases of abdominal adhesions , abdominal pain seven cases , infection in 2 patients in the control group were significantly more complications in the observation group （ P ＆lt;0.05）.Conclusions Compared with open surgery for benign ovarian cysts comparison of clinical effects , laparoscopic surgery with less trauma , less bleeding , rapid postoperative recovery , fewer complications , should be popularized and applied .
