国家基本药物制度对社区卫生服务机构发展及患者满意度影响的调查分析

Investigation and Analysis of the Effects of National Essential Medicine System on the Development of Community Health Service Institution and Patient's Satisfactory Degree

目的:为促进社区卫生服务机构的发展提供参考。方法:采取问卷调查的方式,选择天津市5家社区卫生服务机构和就诊的300名患者进行调查,分析其业务收入、门急诊量、患者对基本药物的满意度及影响患者到社区卫生服务机构就诊的因素,并采用SPSS 13.0统计软件进行分析。结果:2010-2012年,受访机构的业务收入不断增加,门急诊量呈逐年上升趋势,药占比201年大于2010年,而2012年出现下降;受访机构对慢性病的管理水平有所提高;患者对社区卫生服务机构实施国家基本药物制度的满意度较高。经分析,用药合理性、药品价格和医务人员技术水平是影响患者到社区卫生服务机构就诊的关键因素。结论:实施国家基本药物制度对社区卫生服务机构发展有推动作用,同时也提升了患者满意度。今后应进一步提高基本药物的使用频度,遏制各项检查费用的快速增长,并进一步控制药品收入所占的比重,以促进社区卫生服务机构的长效发展。

OBJECTIVE： To provide reference for the development of community health service institutions. METHODS： Ques-tionnaire survey was conducted among 5 community health service institutions and 300 patients from Tianjin. The business income, the change of outpatient capacity, patient＇s satisfactory degree on essential medicine, influential factors of the patient visiting com- munity health service institutions were investigated and analyzed statistically using SPSS13.0 statistical software. RESULTS： During 2010-2012, the income of community health service institutions was growing; outpatient and emergency capacity was rising year by year, and ratio of drugs was higher in 2011 than in 2010, while decreased in 2012; the management of chronic diseases in com-munity health institutions had been improved; patients were more satisfied with essential medicine system in community health ser-vice institutions. Key influential factors of the patients visiting community health service institutions included rationality of drug use, drug price and technical level of medical staff. CONCLUSIONS： The implementation of National Essential Medicine System promotes the development of community health service institutions and improves patient＇ s satisfactory degree. The application of es-sential medicines should be further improved, the rapid increase of inspection charge should be inhibited and the proportion of drug income also should be controlled in order to construct long-term mechanism of the development of community health service institu-tions.
