B型钠尿肽等相关指标在病毒性心肌炎患儿机体的变化研究 被引量:9

摘要 目的对病毒性心肌炎患儿B型脑纳肽(B-type natriuretic peptide,BNP)、左心功能(EF)、C-反应蛋白(CRP)及基质金属蛋白酶-2(MMP-2)的变化进行分析,探讨以上指标与病情严重程度之间的关系。方法 2013年1-7月随机选取医院40例病毒性心肌炎患儿为心肌炎组,同时随机选取30名健康儿童作为对照组,对比两组EF、BNP、CRP、MMP-2的差异;同时根据心肌炎严重程度将心肌炎组分为轻症组28例和重症组12例,对比两组患儿EF、BNP、CRP、MMP-2差异,数据采用SPSS19.0进行分析。结果心肌炎组EF明显低于对照组,而BNP、CRP、MMP-2则明显高于对照组;同时重症组较轻症组,EF更低,BNP、CRP、MMP-2更高;患儿病毒包括脊髓灰质炎病毒6株占15.0%、埃可病毒6株占15.0%、柯萨奇病毒8株占20.0%、流感病毒6株占15.0%、腺病毒6株占15.0%、麻疹病毒4株占10.0%、副流感病毒4株占10.0%。结论病毒性心肌炎患儿EF、BNP、CRP、MMP-2均较健康儿童差,同时随着病情加重,其程度越差。 OBJECTIVE To analyze changes of left ventricular function (EF), B-type natriuretic peptide (BNP), C- reactive protein (CRP) and matrix metalloproteinase-2 (MMP-2) in children with viral myocarditis and explore the relationship between the above indexes and illness severity. METHODS A total of 40 children with viral myocardi- tis during Jan. -Jul. 2013 were randomly selected as the myocarditis group and 30 healthy children as the control group. Differences in EF, BNP, CRP, and MMP-2 were compared between the two groups. The myocarditis group was subdivided to the mild group (28 children) and the severe group (12 children) according to the severity, and differences in EF, BNP, CRP, MMP-2 were compared between these two groups. Data were analyzed using SPSS 19.0. RESULTS EF in the myoearditis group was obviously lower than in the control group, while BNP and CRP, MMP-2 were significantly higher than in the control group. The severe group had lower EF and higher BNP, CRP, MMP-2 compared to the mild group. Patients included 6 patients with poliovirus (15%), 6 patients with echovirus (15%), 8 patients with coxsackie virus (20%), 6 patients with influenza virus (15%), 4 patients with adenovirus (15%), 4 patients with measles virus (10%), and 4 patients with parainfluenza virus (10%). CONCLUSION EF, BNP, CRP, MMP-2 were poorer in children with viral myocarditis compared with healthy children, and the severer the disease, the poorer the level of these indexes.
