免疫功能正常的青年海绵窦‐眶尖曲霉菌感染一例报道并文献复习

Invasive aspergillosis involving cavernous sinus and orbital apex in an immunocompetent youth:a case report and literature review

目的探讨1例免疫功能正常的青壮年颅内曲霉菌感染的临床特点,结合相关文献分析该类人群罹患本病的临床特点。方法收集1例具有强烈嗜血管性的颅内海绵窦‐眶尖曲霉菌感染的免疫功能正常的青年患者的临床资料及诊疗经过,并复习相关文献。结果 22岁青年,无明确免疫异常病史,表现为头痛、视力下降及眼睑下垂,病情凶险,进展迅速,同时出现脑梗死与蛛网膜下腔出血,使用伏立康唑等药物抗真菌治疗无效,最终死亡。尸检提示曲霉菌感染。结论免疫功能正常的青壮年人群也可能罹患颅内真菌感染,早期诊断困难,预后差。

Objective To explore the clinical characteristics in an immunocompetent youth who suffered from intracranial invasive aspergillosis and improve the level to understand and diagnose this disease in clinic. Methods We reported an immunocompetent young patient who developed cavernous sinus and orbital apex aspergillosis and systematically reviewed literatures. Results Twenty-two year-old patient, with no obvious immunoincompetent risks, presented headache, impaired vision, ptosis, cerebral infarction and subarachnoid hemorrhage. Autopsy showed aspergillus infection. Conclusions Immunocompetent young people can also suffer from invasive aspergillosis, early diagnosis and treatment are important to prognosis.
