2012年湖南省二级及以上综合医院抗菌药物临床应用指标分析

Analysis on Clinical Antibiotic Use Indicators in Grade 2 and above General Hospitals in Hunan Province in 2012

目的:了解2012年湖南省二级及以上综合医院抗菌药物临床应用的基本情况,为制定抗菌药物管理政策、促进临床合理应用抗菌药物提供参考依据。方法:从湖南省临床应用监测网采集我省二级及以上综合医院2012年抗菌药物临床应用主要指标,采用Excel表格统计各医院原始数据,进行分类统计、分析。结果:参照卫生部2012年抗菌药物专项整治活动的相关要求,我省监测网所监测的医院包括29所三级综合医院和105所二级综合医院。三级综合医院和二级综合医院的住院患者抗菌药物使用率达标率分别为75.8%、50.5%;抗菌药物使用强度达标率分别为55.2%、18.1%;Ⅰ类切口手术抗菌药物预防性使用率达标率分别为41.4%、38.2%;门诊抗菌药物处方比例达标率分别为75.9%、37.5%;急诊抗菌药物处方比例达标率分别为82.8%、76.3%。结论:我省开展抗菌药物临床应用专项整治工作取得了显著成效,二级及以上综合医院2012年抗菌药物临床应用各指标较2011年均有较大改善,达标率均有提高;三级综合医院抗菌药物整治的效果明显优于二级综合医院。

OBJECTIVE ：To investigate the clinical application of antibiotics in grade 2-above hospitals in Hunan province during 2012 for references of formulating management policy on antibiotics and promoting clinical rational use of antibacterials. METHODS： The antibiotic use indicators in grade 2 and above hospitals in Hunan province in 2012 from were collected from Clinical application monitorinz network, with raw data analyzed statisticallv usln~ Excelspreadsheet. RESULTS： In accordance with the special rectification activity on antibiotic use in 2012 issued by ministry of health, 29 grade-3 general hospitals and 105 grade-2 general hospitals were monitored by Hunan clinical application monitoring network. In grade-3 general hospitals vs. Grade-2 general hospitals, the antibiotic use indicators were up to the standard in 75.8% vs. 50.5% ; the antibiotic use density was up to the standard in 55.2% vs. 18.1% ; the antibiotic use indicators in type I incision patients were up to the standard in 41.4% vs. 38.2% ; the outpatient antibiotic use indicators were up to the standard in 75.9% vs. 37.5%; the emergency antibiotic use indicators were up to the standard in 82. 8% vs. 76. 3%. CONCLUSION： The special rectification activity on antibiotic use carried out in Hunan province has achieved remarkable outcome. The main indicators of antibiotic use in the grade-2 above general hospitals showed great improvement with high rate of up to the standard. The rectification efficacy was significantly better in grade-3 hospitals than in grade-2 general hospitals.
