壮医目诊诊断子宫肌瘤技术规范研究

Technical Specification Research of Diagnosing Hysteromyoma by Eye Observation in Zhuang Medicine

目的:开展壮医目诊诊断子宫肌瘤技术规范化研究。方法:通过壮医目诊观察患者左、右眼"白睛"(巩膜)和"黑睛"(虹膜)相应反映区所获得的资料,采用六步积分法进行评分及诊断,并与金标准比较。结果:102例疑似子宫肌瘤患者经B超确诊为子宫肌瘤的患者97例,壮医目诊出现阳性眼征为88例,灵敏度为90.72%,假阴性率为9.28%,特异度为80.00%,假阳性率为20.00%,阳性似然比为4.54,阴性似然比为1∶8.62,正确诊断指数为70.72%。结论:壮医目诊是壮医具有代表性的特色诊法,具有简便、迅速、安全、准确、便于临床推广等特点,尽管该法诊断子宫肌瘤仍有一定的漏诊率和误诊率,但可作为一种普查方法,尤其是在基层有推广应用价值。并可与B超、CT等检查方法结合应用,对早期发现、及时处治子宫肌瘤具有重大意义。

Objective： To develop the study on diagnosing hysteromyoma by eye observation of Zhuang Medi cine. Method： With the data obtained from observe the corresponding reaction area of sclera and iris of the pa tients＇ two eyes by Eye Observation. To scores and diagnosed by Using Six -Steps scoring method, and then compared with gold standard. Result： From 102 suspected Cases of hysteromyoma, 97 patients with hysteromyo ma were diagnosed by B - ultrasound, and 88 cases showed positive eye sign by eye observation of Zhuang Medi- cine, the sensibility was 90.72% ,the false negative rate was 9.28%, the specificity was 80.00%, the false negative rate was 20.00%, the positive likelihood ratio was 1：4.54, the positive likelihood ratio was 1 ： 8.62, and the youden＇s index is 70.72%. Conclusion ： Eye observation of Zhuang Medicine is a representative charac teristic diagnosis method. This method is easy, simple, rapid, safe, accurate, and suitable for clinical apply. Therefore, eye observation of Zhuang Medicine can be used as a quick diagnose hysteromyoma investigate meth od, especially in basic level hospital, even though loss diagnostic rate and false diagnostic rate still exist in some certain. Eye observation of Zhuang Medicine can be Combinative applied with B -ultrasound and CT, early di agnosis and early detection have great significance in prevention and treatment of hysteromyoma.
