耳后推进皮瓣结合同侧耳甲腔软骨移植修复耳轮全层缺损

Congenital median cleft of lower lip and mandible： surgical treatment and literature review

目的探讨应用耳后推进皮瓣结合同侧耳甲腔软骨移植修复单侧部分耳全层缺损的方法和效果。方法本组10例单侧耳轮缺损患者，其中9例为外伤性耳缺损，1例为冻伤后耳缺损。手术行耳甲腔软骨游离移植，耳后推进皮瓣I期修复，耳轮、耳舟成形术。4例患者行Ⅱ期耳颅角成形术：耳颅沟松解、取对侧耳后皮片植皮术。结果10例患者切口均I期愈合。术后随访3—20个月，耳轮、耳舟形态良好，颜色质地接近，无瘢痕增生。结论对于部分耳轮缺损，应用耳后推进皮瓣结合同侧耳甲腔软骨移植安全、有效。部分患者需要通过Ⅱ期手术来改善双侧耳颅角对称性。

Objective To investigate the usage and effect of postauricular advancement flap and conchal cartilage on the surgical treatment of helix defect. Methods Ten cases with unilateral partial ear defects were treated by postauricular advancement flap combined with ipsilateral conchal cartilage graft. There were 8 males and 2 females, in which 9 cases were caused by injury and 1 case was caused by hot water. Results All cases were followed up for 3 to 12 months with satisfactory results of helix and scapha with similar color. No hyper- trophic scar was reported. Conclusion Repair with postauficular advancement flap combined with ipsilateral conchal cartilage is an effective way for helix defect. Two-stage surgery is possible to achieve a symmetrical cranioauricular angle.
