摘要
目的观察腹腔镜手术联合促性腺激素释放激素激动剂治疗子宫内膜异位症伴不孕症的临床效果及对卵巢功能的影响。方法子宫内膜异位症伴不孕症患者98例,按随机数字表法分为两组:对照组49例,采用单纯腹腔镜手术治疗;观察组49例行腹腔镜手术后加服孕三烯酮胶囊。随访1年,对比两组患者妊娠率、输卵管通畅率和复发率及治疗前后血清卵泡刺激素(FSH)、雌二醇(E2)水平。结果治疗后3个月观察组血清FSH、E2水平明显低于治疗前(P<0.05),且低于对照组(P<0.05)。观察组临床疗效好于对照组(P<0.05)。腹腔镜术后12个月,观察组输卵管再通率为83.7%,妊娠率51.0%,均高于对照组的53.1%、24.5%(P<0.05)。结论腹腔镜手术联合孕三烯酮治疗子宫内膜异位症伴不孕症能有效改善输卵管功能,提高妊娠率,效果优于单纯腹腔镜手术治疗。
Objective To investigate the clinical effect of laparoscopic surgery combined with gonadotrophin releasing hormone agonist on endometriosis patients with infertility and its influence on the patients ′ovarian function . Methods Ninety-eight endometriosis patients with infertility were divided into observation group and control group according to the random number table .Forty-nine cases in the control group received laparoscopic surgical treatment alone,49 cases in the observation group were given gestrinone besides laparoscopic surgery .The follow-up lasted for one year .The pregnancy rate ,fallopian tubal patency rate ,recurrence rate ,serum levels of follicle-stimulating hormone （ FSH） and estradiol （ E2 ） were compared between two groups .Results The serum levels of FSH and E 2 three months after treatment were significantly lower than those before treatment （ P 〈0.05 ） , which were lower than the controls （ P〈0 .05 ） .The clinical effect in the observation group was superior to that in the control group （ P〈0 .05 ） .Twelve months after the patients had received laparoscopic surgery ,the fallopian tubal recanalization rate and pregnancy rate in the observing group were higher than the controls （83.7%vs.53.1%,51.0%vs.24.5%,P〈0.05）.Conclusion Laparoscopic surgery combined with gestrinone can effectively improve the function of fallopian tube , increase the pregnancy rate among endometriosis patients with infertility .Its effect is superior to that of laparoscopic surgical treatment alone.
