早产儿血清NSE水平、NBNA评分及其与分娩胎龄、出生体重的相关性分析 被引量:5

Analysis on the correlation between serum NSE level,NBNA score and gestational age of delivery,birth weight of premature infants

摘要 目的:探讨早产儿血清神经元特异性烯醇化酶(NSE)水平、新生儿20项行为神经评定(NBNA)评分及其与分娩胎龄、出生体重的相关性,为早产儿预后评估提供依据。方法:选取胎龄≤37周的早产儿50例为实验组,同期分娩的健康足月新生儿50例为对照组。研究对象均在生后48 h内采血查血清NSE水平,纠正胎龄足月后15天进行NBNA评估。结果:实验组血清NSE水平高于对照组,差异有统计学意义(t=5.23,P<0.05)。实验组NBNA评分低于对照组,差异有统计学意义(t=-14.54,P<0.05)。早产儿血清NSE水平与NBNA评分(r=-0.368,P<0.05)、出生体重(r=-0.263,P<0.05)、分娩胎龄(r=-0.284,P<0.05)均呈负相关。NBNA评分与出生体重(r=0.578,P<0.05)、分娩胎龄(r=0.742,P<0.05)均呈正相关。结论:血清NSE水平及NBNA评分均是评估早产儿脑损伤的敏感指标。低出生体重和早产是异常NBNA评分的高危因素。 Objective :To explore the correlation between sernm neuron -specific enolase (NSE) level, neonatal behavioral neuro- logical assessment (NBNA) score and gestational age of delivery, birth weight of premature infants, provide a basis for predicting the progno- sis of premature infants. Methods : Fifty premature infants whose gestational age of delivery was 37 weeks or less than 37 weeks were selected as experimental group, and fifty healthy full - term neonates born during the same period were selected as control group ; the serum specimens of the cases in the two groups were abstracted within 48 hours after birth to detect serum NSE levels; NBNA was carried out after correcting gestational age for 15 days. Results: The serum NSE level in experimental group was statistically significantly higher than that in control group (t = 5.23, P 〈 0. 05 ). NBNA score in experimental group was statistically significantly lower than that in control group (t = - 14. 54, P 〈 0. 05 ). There was a negative correlation between serum NSE level and NBNA score ( r = - 0. 368, P 〈 0. 05 ), birth weight ( r = - 0. 263, P 〈 0. 05 ), gestational age of delivery ( r = - 0. 284, P 〈 0.05 ). NBNA score was positively correlated with birth weight ( r = 0. 578, P 〈 0. 05 ) and gestational age of delivery ( r -- 0. 742, P 〈 0. 05). Conclusion : Both serum NSE level and NBNA score are sensi- tive indicators to reflect cerebral injury of preterm infants. Low birth weight and premature birth are high risk factors of abnormal NBNA.
