放射性血管性介入治疗与腹腔镜下保守治疗异位妊娠的对比分析

Comparative analysis of radioactive vascular interventional therapy and conservative treatment under laparoscope in ectopic pregnancy

目的探讨放射性血管性介入治疗与腹腔镜下保守治疗未破裂型异位妊娠的临床效果。方法选择本院治疗的未破裂型异位妊娠患者106例,随机分为观察组和对照组,每组53例,对照组采用腹腔镜保守治疗,观察组采用放射性血管性介入治疗,观察两组的临床疗效。结果两组患者治疗成功率、术后血β-HCG降至50%时间和术后血β-HCG降至正常时间差异无统计学意义(P>0.05)。对患者随访1年,两组术后3个月患侧输卵管通畅率、术后1年内再次妊娠率、再次同侧异位妊娠发生率差异无统计学意义(P>0.05)。结论放射性血管性介入治疗未破裂型异位妊娠的效果及对远期妊娠的影响与腹腔镜保守治疗的效果相当,该方法具有微创、低麻醉风险的特点,能够有效保留患者的生育功能,值得临床推广应用。

Objective To discuss the efficacy of radioactive vascular interventional therapy and conservative treatment under laparoscope in unruptured ectopic pregnancy.Methods One hundred and six patients with unruptured ectopic pregnancy admitted into our hospital were evenly divided into observation group （53 cases） and control group （53 cases） in random.In the control group,the laparoscopic conservative treatment was provided,while in the observation group,the radioactive vascular interventional therapy was adopted.The efficacy in both groups was observed.Results The success rate of treatment,time of 13-HCG decreasing to 50% in blood after treatment,and time of [3-HCG decreasing to be nor- mal between two groups had no significant difference （P〉0.05）.After one-year follow-up,tubal patency rate of the af- fected side 3 months after treatment,rate of a second time pregnancy one year after treatment,and incidence of a second time ectopic pregnancy in the same side in both groups had no significant difference （P〉0.05）.Conelusion The curative efficacy of unruptured ectopic pregnancy treated by radioactive vascular interventional therapy and influence on long- term pregnancy are comparable to those by laparoscopic conservative treatment.The former has advantages of minimal invasion,low risk of anesthetization,and effectively keeping reproductive function,which is worthy of expansion in clinic.
