腹腔镜手术中采用“腹腔镜下双J管置入器”放置双J管:附视频

The experiences of double J tube placement by laparoscopic double J tube inserter in laparoscopic surgery

目的总结和介绍应用专利产品"腹腔镜下双J管置入器",在腹腔镜手术中放置双J管的经验及技巧,评价其临床价值。方法 45例患者接受腹腔镜下输尿管切开取石术,术中均使用双J管专用置入器,将双J管置入输尿管中。分析该操作的完成时间、成功率、并发症以及相应的处理。结果本组腹腔镜手术45例,44例术中置管顺利,放置双J管所需时间30 s^7 min,平均56 s。双J管放置准确到位的42例,双J管近端未进入肾盂2例。1例置管阻力较大,远端未能进入膀胱,后改截石位,输尿管镜检查发现双J管远端穿出输尿管腔外,输尿管镜下取出双J管,重新放置。术后无漏尿等并发症发生。结论腹腔镜下利用专利产品置入器放置双J管,方法简单,效果确切,值得在临床推广应用。

Objectives To summarize and introduce our initial experiences with the patented product laparoscopic double J tube inserters during laparoscopic surgeries, and assess the value in clinic. Methods The laparoscopic double J tube inserters were applied in 45 patients undergoing retroperitoneoscopic ureterolithotomy to place the double J tube into ureter. The completion time, success rate, complications and corresponding measurements would be analyzed in details. Results Among 45 cases, the procedures of 44 patients were successfully done. The time of double J tube insertion ranged from 30 s to 7 rain, of which the mean time was 56 s. The insertions of 42 cases were accurately placed while the proximal end of double J tube failed to be inserted into renal pelvic in 2 cases. Only 1 case failed to be inserted the distal end of double J tube into the bladder because of great resistance. The double J tube had been turned out to pierce out of ureter examined by ureteroscope and was inserted into ureter successfully for the second time. No postoperative complications, such as urine leakage, were observed. Conclusions It was an effective and efficient method to use patented inserter to place double J tube, which can be widely used clinically.
