经尿道前列腺电切除术治疗前列腺增生合并膀胱逼尿肌无反射的临床疗效观察

Clinical observation of TURP in the dysfunction of detrusor following partial outlet obstruction

摘要：目的探讨经尿道前列腺电切除术（TuRP）治疗有膀胱逼尿肌无反射的良性前列腺增生（BPH）并发膀胱出口梗阻（BOO）的临床疗效。方法回顾分析2008年5月～2011年8月该科23例经尿流动力学检查确诊有膀胱逼尿肌无反射的前列腺增生患者的临床资料。对23例确诊为前列腺增生并发逼尿肌无反射的患者行TURP。手术前和术后12个月分别记录国际前列腺症状评分（IPSS）、最大尿流率时逼尿肌压（Pdet）、最大尿流率（Qmax）、残余尿量（pvp．），并进行手术前后比较，分析综合治疗疗效。结果23例良性前列腺增生合并逼尿肌无反射患者治疗前后IPSS评分、最大尿流率时逼尿肌压、最大尿流率、残余尿量明显改善。结论TURP术对BPH合并逼尿肌无反射患者的临床疗效是确切的。

[ Objective ] To evaluate the clinical effect of transurethral resection of prostate （TURP） for benign prostatic hyperplasia （BPH） patients with detrusor areflexia and bladder outlet obstruction （BOO）. [Methods ] The clinical data of 23 BPH patients who were diagnosed as detrusor areflexia were retrospectively analyzed. All the pa- tients accepted TURP surgery. The preoperative and 12 months postoperative clinical index including international prostate symptom score （IPSS）, free urinary flow rate （Qmax） and residual urine volume （PVR） of the patients were recorded and statistically analyzed. [Results ] Compared with the preoperative clinic index, the IPSS, Qmax and PVR of the patients were significantly improved 12 months after TURP surgery. [ Conclusion ] TURP surgery is ef- fective for BPH patients with detrusor areflexia.
