快速康复外科理念在胃癌围术期护理中的应用

Application of fast track surgery in the perioperative nursing care of surgical patients with gastric cancer

目的:探讨快速康复外科(FTS)理念在胃癌围术期护理中的应用效果。方法:将128例胃癌根治术患者随机分为观察组和对照组各64例,对照组实施围术期常规护理措施,观察组接受FTS理念指导下的针对性护理干预措施。比较两组患者护理干预前后血清白蛋白、IgA水平,胃肠道功能恢复时间、肛门排气时间,负面情绪评分等。结果:术后1 d、3 d,两组患者白蛋白及IgA水平均低于手术前(P<0.05),观察组白蛋白及IgA水平明显高于对照组(P<0.05);观察组患者术后肠鸣音恢复时间及肛门排气时间均显著短于对照组患者(P<0.05);护理干预后两组患者负面情绪水平均低于护理前(P<0.05),且观察组患者HAMD评分及HAMA评分均低于对照组(P<0.05)。结论:以FTS理念为指导的护理干预可以有效优化胃癌患者术后体质并加速康复进度,降低患者焦虑、抑郁等不良情绪。

Objective：To investigate the application effect of fast track surgery （ FTS） in the perioperative nursing care of surgical pa-tients with gastric cancer .Methods：128 patients undergoing radical gastric cancer surgery were randomly divided into the observation group and the control group （64 cases in each group ） .The conventional perioperative nursing care was implemented in the control group and the corresponding nursing intervention under the guidance of the FTS concept was provided for the patients in the observation group .The level of serum albumin and IgA of the patients ,the recovery time of gastrointestinal function , anal exhaust time and the score of negative emotion were compared between the two groups before and after nursing intervention .Results：The level of serum albumin and IgA of the patients was lower on the first and third day after operation than that before operation in the two groups （ P〈0.05）;the level of serum albumin and IgA of the patients was obviously higher in the observation group than the control group （P〈0.05）;the postoperative recovery time of bow-el sound and anus exhaust time were significantly shorter in the observation group than the control group （ P〈0.05）;the patient＆#39;s negative emotion level was lower after nursing intervention than that before the intervention （P〈0.05）;the HAMD score and HAMA score of the patients was lower in the observation group than the control group （P〈0.05）.Conclusion：The nursing intervention under the guidance of the FTS concept can effectively optimize patient＆#39;s physical status and accelerate the progress of rehabilitation and alleviate patient ＆#39;s anxiety and depression .
