微创经皮肾镜钬激光碎石术与开放性肾切开取石术治疗复杂性肾结石临床疗效比较

Comparative study on clinical effect of minimally invasive percutaneous nephrolithotomy with holmium laser lithotripsy and open renal lithotomy treatment for complex renal calculi

目的探讨微创经皮肾镜钬激光碎石术与开放性肾切开取石术治疗复杂性肾结石的临床疗效。方法116例复杂性肾结石患者随机分为研究组(微创经皮肾镜钬激光碎石术组)和对照组(开放性肾切开取石术组),各58例。采集两组患者临床资料,并对临床治疗效果、指标改善情况、并发症发生情况进行比较和分析。结果与对照组相比,研究组结石清除的比率显著提高(98.27%),而结石残留的比率则显著降低(1.73%),差异有显著性(P<0.05);研究组术中出血量明显减少,手术时间、下床活动时间、住院时间均明显缩短(P<0.05);研究组并发切口感染、器官损伤、出血、败血症、尿外渗的总发生率显著降低(P<0.05)。结论微创经皮肾镜钬激光碎石术治疗复杂性肾结石其临床治疗效果优于开放性肾切开取石术,且并发症发生率低。

[Objective] To compare the clinical effect of minimally invasive percutaneous nephrolithotomy with hohnium laser lithotripsy and open renal [ithotomy treatment for complex renal cah.uli. [Methods ] 116 complex re- nal calculi patients were divided into study group （minimally invasive percutaneous nephrollthotomy with holmium laser lithotripsy group） and control group （open renal lithotomy group） randomly, 58 cases in each group. All clinical data were collected and the clinical treatment situations, improved situations of clinical indicators and occmTence of complications of the patients in two groups were compared and analyzed. [Results] Compared with control group, the patients in study group the stone clearance rate was increased significantly （98.27%）, the residual stone rate was decreased significantly （1.73%）,（P 〈0.05）. The blood loss of intraoperative in study group was reduced significantly. Operative time, ambulation and hospitalization time in study group were shortened significantly （P 〈0.05）. The inci- dence of complication by wound infection, organ damage, hemorrhage, sepsis and urinary extravasation in study gruup were decreased significantly （P 〈0.05）. [ Conclusion] The clinical effect of minimally invasive percutaneous nephrolithotomy with holmium laser lithotripsy was better in the treatment of complex renal calculi with lower inci- dence of complications.
