冠状动脉斑块旋磨术在严重钙化病变介入治疗中的疗效 被引量:7

摘要 目的:评价冠状动脉斑块旋磨术联合药物洗脱支架置入术治疗严重钙化病变的疗效和安全性。方法:入选2011年11月至2013年6月在我院行冠状动脉旋磨术和药物洗脱支架置入术的34例患者,共38处严重钙化病变。观察即刻手术成功率,并进行临床随访。结果 :患者平均年龄(73±12)岁,急性冠脉综合征占91.2%,合并高血压者占88.2%。29.4%的旋磨病变为器械不能通过或球囊不能扩张。24例(70.6%)患者术中仅使用1.5 mm旋磨头,3例患者使用2个旋磨头。旋磨头与血管直径比为0.5±0.1。旋磨术后成功置入68枚药物洗脱支架。术中2例(5.9%)患者发生慢血流现象,经治疗后造影成功率达100%,无冠脉穿孔及紧急冠脉旁路移植术。行血管内超声检查的12例患者中补救性旋磨术比例、介入手术相关心肌梗死发生率较未检查者有降低趋势。术后平均随访9.7个月,1例患者因心衰恶化死亡,无靶血管重建病例。结论:冠状动脉斑块旋磨术联合药物洗脱支架置入术治疗严重钙化病变安全有效,中期随访疗效满意。
