2010～2013年某院中药注射剂使用情况分析及合理性评价

Analysis and evaluation of the utilization of traditional Chinese medicine injections in a hospital from 2010 to 2013

目的分析某院2010～2013年中药注射剂的应用情况及合理性评价，为临床合理用药提供参考。方法通过医院HIS系统调取中药注射剂的用药数据，对某院中药注射剂的用药品种、金额、用药频度（DDDs）等进行描述性统计，分析其用药规律；查阅病历系统及合理用药分析系统，评价中药注射剂的用药合理性。结果中药注射剂的用药金额呈现逐年增长趋势，2010～2013年中药注射剂用药金额分别占全年药品总金额的10．O％、10．1％、10．5％、10．6％，疏血通注射液连续2年跃居DDDs排名第1位；活血化瘀类中药注射剂使用最为广泛，历年占比均超过45％：使用中存在用药剂量过大、联合用药不当、疗程偏长、溶剂选择不当等情况。结论该院中药注射剂使用基本合理，但也存在不规范之处。有待通过临床药师干预、医嘱处方点评、医疗质量监督、合理用药宣教等手段来进一步规范。

Objective To analyze and evaluate the utilization of traditional Chinese medicine injections in a hospital from 2010 to 2013, so as to provide references for clinical rational drug use. Methods The drug utilization data of tra- ditional Chinese medicine injections provided by the hospital＇s HIS system were adopted to compare the consumption sum, types and DDDs by descriptive statistics, in order to find out the rule of drug use. The system of patient records and the system of rational drug use evaluation were reviewed so as to evaluate the rationality of drug use of traditional Chinese medicine injections. Results The consumption sum of traditional Chinese medicine injections increased year by year. From 2010 to 2013, it respectively accounted for 10.0%, 10.1%, 10.5%, and 10.6% of the total annual pricing, and the pricing of Shuxuetong Injection ranked the first place in DDDs for two consecutive years. The Chinese patent injections to promote blood circulation and remove blood stasis were used most frequently in the hospital, accounting for more than 45% throughout the years. As to the use of drugs, problems like over-dosage, inappropriate combination of drug use, prolonged therapy, improper choose of solvent could still be found. Conclusion The utilization of the tra- ditional Chinese medicine injections in the hospital is basically rational. However, some problems still exist, which call for measures like the intervention of clinical pharmacist, review of prescription, monitor of medical quality and popular- ization of rational drug use.
