踝关节骨髓腔水肿MRI表现 被引量:4

摘要 目的探讨踝关节骨髓腔水肿的MRI表现。方法回顾性分析78例(78个踝关节)骨髓腔水肿患者临床及影像学资料,分析踝关节骨髓腔水肿MRI特征。结果 78例中左踝骨髓腔水肿33例,右踝45例;距骨滑车上缘损伤41例(52.6%),距骨颈损伤23例(29.5%)、距骨体损伤18例(23.1%)、胫骨内踝关节面损伤18例(23.1%);MRI显示水肿病灶182处,T2WI压脂、PDWI压脂序列信号较一致,均呈高信号病灶179处(98.4%),均呈低信号病灶3处(1.6%)。结论踝关节骨髓腔水肿MRI检查T2WI压脂及PDWI压脂序列呈不同程度高信号,T1WI序列呈等、低信号,T2WI序列可呈各种信号;对疑诊踝关节骨髓腔水肿患者应及时行MRI检查明确诊断。 Objective To analyze MRI findings of bone marrow edema of ankle. Methods The MRI findings were retrospectively analyzed in 78 patients with bone marrow edema of ankle. Results In 78 patients (78 ankles) w(th bone marrow edema (33 left ankles and 45 right ankles), 41 lesions (52.6%) were located at talus, 23 (29.5%) at talar neck, 18(23. 1%) at talar body, and 18 (23. 1%) at the articular surface of the tibial malleolus. MRI revealed 182 edema lesions, in which 179 edema lesions had high signal intensity on fat-saturated T2-weighted and PD-weighted sequences, and 3 edema lesions had low signal intensity on both two sequences. Conclusions The marrow edema of ankle shows high signal intensity on T2 WI and PDWI, equal and low signal intensity on T1 WI, and various signal intensity on T2 WI, therefore the patients with suspected bone marrow edema should be nerformed MRIscan to confirm the diagnosis.
