甲壳质酶蛋白在支气管哮喘患者血清中的表达及意义 被引量:2

Expression and Significance of YKL-40 in Serum of Patients with Bronchial Asthma

摘要 目的探讨甲壳质酶蛋白(YKL-40)在支气管哮喘患者血清中的表达及意义。方法 42例支气管哮喘患者分为病情未控制组22例和基本控制组20例,18例健康正常成年人为对照组,采用ELISA法检测其血清中的YKL-40、血清总IgE水平,并记录血嗜酸粒细胞计数、FEV1%pred及ACT评分结果。结果未控制组患者血清YKL-40、血清总IgE、嗜酸粒细胞均较基本控制组和对照组升高明显,差异具有统计学意义(P<0.05)。基本控制组与对照组患者各项指标无明显差异性,未控制组患者血清YKL-40升高与血清总IgE的表达无相关性,与血嗜酸粒细胞计数呈正相关,与FEV1%pred、ACT评分呈负相关。结论 YKL-40水平在未控制组哮喘患者血清中升高明显,且与嗜酸性粒细胞升高呈正相关,与肺功能呈负相关,可作为哮喘病情控制程度的一项血清学指标。 Objective To explore the expression and significance of chitinase-like protein ( YKL-40) in serum of patients with bronchial asthma. Methods 42 cases of bronchial asthma, who received treatment in the Second Hospital of Lanzhou University between January and September 2013, were recruited in the study. They were divided into an uncontrolled group ( n = 22) and a well-controlled group ( n =20) . 18 cases of normal healthy adults were regarded as control. Serum YKL-40 and total IgE levels were measured by ELISA. Meanwhile the blood eosinophil count, FEV1% pred and ACTscore were measured and recorded. Results Serum YKL-40, total IgE, and eosinophil in the uncontrolled group significantly increased than those in the well-controlled group and the healthy group( P 〈0. 05) . The differences between the well-controlled group and the healthy group were not significant ( P 〉0. 05) . In the uncontrolled group,the serum YKL-40 level was not correlated with total IgE ( P 〉0. 05) , but positively correlated with blood eosinophil count( r = 0. 826, P 〈 0. 001) , and negatively correlated with FEV1% pred and ACT score ( r =- 0. 607, P =0. 03; r = - 0. 698, P 〈 0. 001) . Conclusions Serum YKL-40 level in uncontrolled asthma patients is significantly elevated, and positively correlated with elevated eosinophils and negatively correlated with lung function, so may be used as a serummarker of asthma control level.
