丙戊酸钠联合奥氮平治疗双相Ⅰ型障碍急性躁狂发作对照研究

A control study of sodium valproate plus olanzapine in the treatment of acute manic episode of bipolar I disorder

目的：探讨奥氮平联合丙戊酸钠治疗双相Ⅰ型障碍急性躁狂发作的临床疗效和安全性。方法将60例双相Ⅰ型障碍急性躁狂发作患者随机分为两组，两组均口服丙戊酸钠治疗，研究组联合奥氮平治疗，观察6周。于治疗前后采用Bech-Rafaelsen躁狂量表评定临床疗效，副反应量表评定不良反应。结果治疗6周末研究组痊愈率69．0％，对照组为17．9％，研究组痊愈率显著高于对照组（χ2＝15．11，P＜0．01）；治疗过程中两组均未出现严重不良反应。结论丙戊酸钠联合奥氮平治疗双相Ⅰ型障碍急性躁狂发作较单用丙戊酸钠起效更快，效果更显著，且安全性高。

Objective To explore the efficacy and safety of sodium valproate plus olanzapine in the treat-ment of acute manic episode of bipolar I disorder .Methods Sixty patients with acute manic episode of bi-polar I disorder were randomly divided into two groups ,both groups took orally sodium valproate ,and re-search group was plus olanzapine for 6 weeks .Efficacies were assessed with the Bech-Rafaelsen Mania Rat-ing Scale （BRMS） before and after treatment and adverse reactions with the Treatment Emergent Symp-tom Scale （TESS） .Results At the end of the 6th week cure rate was significantly higher in research than in control group （69 .0% vs .17 .9% ,χ2 =15 .11 , P〈0 .01）;both groups had no severe adverse reactions during treatment .Conclusion Sodium valproate plus olanzapine takes effects more rapidly and has a more evident effect and higher safety in the treatment of acute manic episode of bipolar I disorder .
