腹腔镜胃癌根治手术并发症的防治

Complications and it＇s prevention strategies of laparoscopic gastrectomy for gastric cancer

目的探讨腹腔镜下远端胃癌根治术的术后并发症及其防治方法。方法对2010年2月至2012年8月期间实施的336例腹腔镜下胃癌根治术进行回顾性分析，观察其术后并发症的发生及防治情况。结果手术并发症的发生率为15．2％，包括皮下气肿11例，切口感染8例，吻合口狭窄5例，吻合口瘘2例，术后1个月肠梗阻8例，术后6个月肠梗阻17例。无因各种原因导致的中转开腹手术，未发生患者术中及术后死亡。结论在度过学习曲线后，腹腔镜胃癌手术是安全的，并不增加手术并发症。

Objective To study the complications and it＇s prevention strategies of laparoscopic gastrectomy for gastric cancer. Methods Between February 2010 and August 2012, 336 patients who received gastrectomy for gastric cancer at our department were studied. Clinical complications and its prevention and cure were observed. Results The incidence of surgical complications was 15.2%, including subcutaneous emphysema ll cases, wound complications 8 cases, 7 cases of anastomotic stricture and fistula, postoperative ileus 8 cases within one month, 17 cases within half one year. No laparotomy results from all causes happened. There is no patients died intraoperative and postoperative. Conclusion Throughout the learning curve, laparoscopic gastric cancer surgery is safe, doesn＇t increase the incidence of complications.
