妊娠期甲亢患者血清甲状腺功能和免疫含量变化及其临床价值

Clinical values of serum thyroid function and immune content of patients with Graves disease in pregnancy

目的探讨处于妊娠期甲亢的患者其甲状腺功能及免疫功能的变化,分析在临床中的应用价值。方法分别测量138例妊娠期甲亢患者和216例非妊娠期甲亢患者在妊娠15、25、35周末的平均血清促甲状腺激素(TSH)、促甲状腺激素受体抗体(TRAb)、总甲状腺素(T4)、游离甲状腺素(FT4)、三碘甲状腺原氨酸(T3)、游离三碘甲状腺原氨酸(FT3)的浓度,比较两组在同一时期的差异,利用统计学方法分析其结果。结果在妊娠期间所有阶段的妊娠期甲亢患者,其血清T3和T4的水平比对照组的水平高(P<0.05);在妊娠25、35周末,妊娠组和对照组的FT3和FT4的差异无统计学意义(P>0.05)。结论妊娠期甲亢的诊断可依据患者血清中FT3、FT4及TRAb的改变,T3和T4的改变没有太大意义。

Objective To discuss the changes of thyroid function and immune function of patients with Graves disease and analyze the application values in clinic. Methods The average concentration of serum TSH,TSH receptor antibody （TRAb）,total thyroid hormone （T4）,free thyroid hormone （FT4）, triiodothyronine （T3） and free triiodothyronine （FT3） at the weekend of 15 week,25 week and 35 week of 138 patients with Graves disease and 216 patients without Graves disease were detected respectively, compared the differences of two groups at the same period,analyzed the results by statistical methods. Results In all phases of pregancy,the level of serum T3 and T4 of patients with Graves disease was significantly higher than that of control group （P＆lt;0.05）. At the weekends of 25 week and 35 week,differences of FT3 and FT4 in two groups were small （P＆gt;0.05）. Conclusion Changes of serum FT3, FT4 and TRAb in patiens with Graves disease in pregancy has diagnostic significance in clinic,and changes of T3 and T4 is insignificant.
