带蒂背阔肌肌瓣局部转位填充乳癌根治术后腋窝组织缺损

Local transfer of pedical flap of Latissimus dorsi to fill up tissue defect of armpit after radical mastectomy

目的 探讨应用带蒂背阔肌肌瓣局部转位填充乳癌根治术后腋窝组织缺损的价值。方法 对 3 1例行Halsted手术病人用带蒂背阔肌肌瓣转位填充腋窝和锁骨下区域。结果 有 1例肌瓣部分坏死 ,其余全部成活。腋窝积液 1例 ,上肢短期轻度水肿 2例 ,皮瓣轻度坏死 2例。结论 该手术能矫正腋窝凹陷畸形 ,减轻术后疼痛、麻木等不适症状 ,减少积液、上肢功能障碍等并发症 ,且手术操作简便 ,是弥补乳癌根治术所带来不足之处的有效术式。

Objective To explore the value of local transfer of pedical flap of latissimus dorsi to fill up tissue defect of armpit after radical mastectomy.Methods In 24 cases of the Halsted operation,pedicle flap of latissimus dorsi Local was transferred and filled up to arpmit and infraclaricular fossa .Results All succeeded except for 1 case of muscle flap having slight necrosis There were complications of 1 case of armpit dropsy,2 cases of upper arm slight edema for a short rerm,2 cases of skin flap slight necrosis.Conclusions Local transfer of pedical flap of Latissimus dorsi to fill up tissue defect of armpit after radical mastectomy can correct malformation of arpmit、and decrease paralysis and pain and so on and reduce complication of dropsy and disfunction of upper arm,to avoid insufficiency of radical masectomy.
