摘要 目的 :探讨不同类型冠心病患者中性粒细胞和单核细胞膜 CD11b/CD18表达的变化。方法 :选择经冠状动脉造影确诊的 49例心绞痛患者 ,30例急性心肌梗死患者和 2 0例正常人 ,用流式细胞仪直接免疫荧光法检测中性粒细胞和单核细胞膜 CD11b/CD18表达。结果 :冠心病患者中性粒细胞和单核细胞膜 CD11b/CD18表达较正常对照组均显著增加 (P<0 .0 1) ;不稳定性心绞痛和急性心肌梗死患者中性粒细胞和单核细胞膜 CD11b/CD18表达显著高于稳定性心绞痛患者 (P<0 .0 1)。与正常对照组比较 ,心绞痛患者组中性粒细胞和单核细胞计数无变化 (P>0 .0 5 ) ,而急性心肌梗死组明显增加 (P<0 .0 1)。急性心肌梗死患者中性粒细胞和单核细胞膜 CD11b/CD18表达与梗死范围无关。结论 :冠心病患者中性粒细胞和单核细胞膜 CD11b/CD18表达明显增加 ,其增加程度与心肌缺血的类型有关。 AIM: To investigate the changes of expression of adhesion molecules CD11b/CD18 on neutrophils and monocytes in different types of patients with coronary heart disease (CHD). METHODS: The expression of CD11b/CD18 on neutrophils and monocytes were measured by direct immunofluorescence techniques associated with flow cytometry in 49 patients with angina pectoris confirmed by coronary arteriography, 30 patients with acute myocardial infarction (AMI) and 20 normal individuals. RESULTS: Expressions of CD11b/CD18 on neutrophils and monocytes in the CHD patients were markedly higher than those in normal cases( P <0.01),and the expressions of adhesion molecules CD11b/CD18 on neutrophils and monocytes in patients with unstable angina pectoris and AMI were significantly higher than those in patients with stable angina pectoris( P <0.01). No change was observed in neutrophil and monocyte counts in patients with angina pectoris ( P >0.05), WBC' counts in AMI patients were markedly higher than those of the normal cases. The expression of adhesion molecules CD11b/CD18 on neutrophils and monocytes in AMI patients bore no relation to increased infarct extent. CONCLUSION: Neutrophil and monocyte CD11b/CD18 adhesion molecules in patients with CHD had a higher expression, and the increased degree of expression might be related to the types of myocardial ischemia.
