应用自体心包瓣置换术治疗主动脉瓣病变

The Aortic Valve Replacement with Autologous Pericardial Heart Valve

目的 报告无支架自体心包瓣置换主动脉瓣手术的临床应用效果。 方法 11例单纯主动脉瓣病变患者行自体心包瓣置换主动脉瓣手术 ,术后定期随访。 结果 11例均存活 ,顺利出院 ,随访时间 5～ 30个月 ,平均随访时间 2 4.2± 5 .6个月。 1例因中等量反流而行二次手术 ,其余 10例心包瓣膜功能良好。 结论 该手术是一种治疗单纯主动脉瓣病变较为理想的方法 ,术后患者无需长期抗凝 ,手术近期效果满意 ,远期效果有待进一步随访。

Objective\ To report the clinical results of aortic valve replacement with autologous pericardial heart valves(APHV).\ Methods There were 11 patients with simple aortic valve disease with ages ranging from 19 to 59 years(mean 39.2±13.7 years), who underwent aortic valve replacement with APHV. All patients were followed up clinically at 3 months and 6 months and every year subsequently by examination of echocardiography and electrocardiography after operation.\ Results\ Eleven patients survived without major events. follow up ranged from 5 to 30 months(mean 24.2±5.6 months). One patient required reoperation as a result of aortic regurgitation at 5 months after operation. The performed function of APHV of other 10 patients was well. Left ventricular volume was significantly reduced and cardiac function was improved greatly at 6 months after operation. No patients had complication of angina, endocarditis, thromboembolia and hemorrhage events during followed up.\ Conclusion This technique is an ideal method of treatment simple aortic valve disease. Early results were excellent because of significant recovery of left ventricular function and volume, and not requiring of anticoagulation. Long term results should be followed up.
