新生儿肺透明膜病的临床与X线分析

Clinical and X-ray Analysis of Hyaline Membrane Disease in Newborns

目的分析新生儿肺透明膜病(HMD)的临床与X线特点,提高对新生儿HMD的认识与X线诊断水平。方法收集2010年2月至2012年8月延安市人民医院收治的新生儿HMD患儿28例,摄取仰卧前后位X线胸片,在治疗过程中复查X线片。结果根据X线表现,本组28例患儿中新生儿HMDⅠ级7例,Ⅱ级10例,Ⅲ级9例,Ⅳ级2例。在治疗过程中出现并发症10例,其中肺不张2例,气胸3例,肺炎2例,缺血缺氧性脑病1例,坏死性肠炎2例。结论新生儿HMD有一定的X线特征,是及时了解病程发展与治疗效果的主要手段。

Objective To analyze the clinical and X-ray characteristics of hyaline membrane disease （HMD） in newborns ,to improve the knowledge and X-ray diagnosis level of HMD in newborns. Methods A total of 28 cases of neonatal pulmonary HMD in newborn admitted to Yan＇an People＇s Hospital from Feb. 2010 to Aug. 2012 were collected ,the supine anteroposterior chest X-ray was taken, and the X-ray film was reexamined in the course of treatment. Results According to X-ray manifestations, the HMD newborns included 7 cases with degree Ⅰ changes, 10 cases with degree Ⅱ changes,9 cases with degree Ⅲ changes, and 2 cases with degree Ⅳ changes. Complications in 10 cases occurred during the treatment, including 2 cases of atelectasis,3 cases of pneumothorax,2 cases of pneumonia,1 case of hypoxic-isehemic encephalopa- thy, and 2 cases of necrotizing enterocollitis. Conclusion X-ray manifestations of HMD in newborn are of certain characteristics and it is the main method to understand the disease progress and therapeutic effect.
