微创经皮锁定钢板内固定治疗Pilon骨折疗效分析

Curative Effect Analysis of Internal Fixation with Locking Plate and Minimally Invasive Percutaneous Plate Osteosynthesis in Treatment of Pilon Fractures

目的观察微创经皮锁定钢板内固定治疗Pilon骨折的临床疗效。方法选择2009年1月至2012年12月台山市人民医院骨科收治的120例Pilon骨折患者作为研究对象,按照随机数字表法将患者分为两组,每组60例,内固定治疗组采用微创经皮锁定钢板内固定治疗,手术切开复位组采用传统手术切开复位钢板内固定治疗,比较两组患者手术时间、术中出血量、骨折愈合时间以及术后12个月两组患者的疗效。结果 120例患者均成功完成手术,内固定治疗组手术时间、术中出血量、骨折愈合时间均显著少于手术切开复位组(t=4.074、6.001、4.481,P<0.01);术后12个月内固定治疗组的疗效显著优于手术切开复位组(z=4.682,P<0.05)。结论微创经皮锁定钢板内固定治疗Pilon骨折的手术时间短、术中出血少、骨折愈合时间短,且疗效显著,值得推广应用。

Objective To observe the curative effect of internal fixation with locking plate and minimally invasive percutaneous plate osteosynthesis in treatment of Pilon fractures. Methods Total of 120 patients with Pilon fracture admitted to Taishan City People＇s Hospital from Jan. 2009 to Dec. 2012 were selected as study objects. The 120 cases were divided into two groups by random number table method,60 cases in each group, the internal fixation group were treated with internal fixation with locking plate and minimally invasive percutaneous plate osteosynthesis, the open reduction group were treated with traditional operation of open reduction and internal fixation. The operation time, amount of bleeding of fracture operation and healing time, the curative effect of patients at 12th month after operation were compared. Results All of the 120 patients completed the operation successfully, the operation time, bleeding of fracture operation and healing time of internal fixation were less than that of the open reduction group （ t = 4. 074,6.001,4.431, P 〈 0.01 ）. After 12 months, the effect of the internal fixation group was better than that the open reduction group （z = 4. 682, P 〈 0.05 ）. Conclusion Internal fixation with locking plate and minimally invasive percutaneous plate osteosynthesis has short time of operation,little bleeding, short healing time and significant effect for treatment of Pilon fractures,thus is worthy of popularization.
