血必净联合喜炎平治疗重症胸腹损伤急性凝血功能障碍的研究

Study on the treatment with Xuebijing combined with Xiyanping for acute coagulation disorders during sever thoracoabdominal injuries

目的探讨血必净联合喜炎平干预对重症胸腹损伤急性凝血功能障碍的治疗效果。方法重症胸腹损伤患者129例随机分为2组,均给予重症胸腹损伤常规处理与治疗,干预组在此基础上给予血必净注射液和喜炎平静脉滴注,2组在就诊及入院时分别检查血小板计数(Plt)、血浆D-二聚体(D-D)、部分活化凝血酶原时间(APPT)、凝血酶原时间(PT)、凝血酶时间(TT)、肿瘤坏死因子-α(TNF-α)、内毒素(LPS)、白细胞介素-6(IL-6)、磷脂酶A2(PLA2)和血小板活化因子(PAF),观察2组上述指标变化情况。结果干预组患者干预后Plt升高,D-D、APTT、PT、TT降低,TNF-α、LPS、IL-6、PLA2、PAF下降,与干预前及对照组相比差异有统计学意义(P均<0.01)。结论血必净注射液联合喜炎平可改善重症胸腹损伤急性凝血功能障碍,对改善预后、提高生存率有重要意义。

Objective It is to explore the effect of intervention of Xuebijing （XBJ）combined with Xiyanping on acute co- agulation disorders during sever thoracoabdominal injuries. Methods 129 patients with sever thoracoabdominal injury were ran- domly divided into two groups： invention group and control group. Both groups were given normal therapy for sever thoracoab- dominal injuries, and the invention group was given XBJ and Xiyanping by intravenous drip infusion. Platelet count （Plt）, D - dimer（ D - D） , activated partial thromboplastin （APPT） , prothrombin time （PT） , thromboplastin time （TT） , tumor necrosis factor-α（ TNF -α） , lipopolysaccharide （LPS） , interleukin - 6 （ IL - 6 ） , phospholipase A2 （ （ PLA2 ） and platelet-activating factor（PAF） were determined and compared in both groups at the time of arriving at the emergency department and at the time of leaving the emergency department to the inpatient department. Results After intervention, the level of Pit increased, and that of D - D, APTT, PT, TT, TNF -α, LPS, IL - 6, PLA2, PAF decreased, compared with that before intervention and control group, the difference was significant（P 〈 0.01 ）. Conclusion XBJ combined with Xiyanping can improve acute coagu- lation disorders during sever thoracoabdominal injuries, thus it has an important significance for improving prognosis and in- creasing survival rate.
