Pharmaceutical care for one hepatolenticular degeneration patient

通过参与肝豆状核变性患者的药学实践,探讨临床药师开展药学监护的方法。方法:以1例肝豆状核变性患者的诊治过程为例,临床药师参与评估药物治疗方案,并对患者进行药学监护和用药教育。结果:临床药师通过药学监护,发现患者用药中存在问题,通过用药教育提高了患者药物治疗的有效性、安全性和依从性。结论:临床药师通过药学监护和用药教育,能提高临床药物治疗水平,从而在治疗团队中发挥积极作用。

Purpose ： exploring the method of pharmaceutical care by clinic pharmacists by taking part in the pharmaceutical practice of hepatolenticular degeneration patient. Method： taking the treatment process of one hepatolenticular degeneration patient for example, clinic pharmacists took part in the evaluation of pharmaceutical treatment project and gave pharmaceutical care and instruction to the pa- tient. Results： by pharmaceutical care, the clinic pharmacists found out the problems in the medicine use and improved the effective- ness, safety and dependence of pharmaceutical treatment. Conclusion： clinic pharmacists/pharmaceutical care and instruction could im- prove pharmaceutical treatment and plaved an important role in the treatment axoup.
