栓塞治疗输卵管积水对体外授精-胚胎移植结局的影响

The effect of interventional embolization treatment for hydrosalpinx on the outcome of in vitro fertilization and embryo transfer

目的探讨输卵管积水以及通过介入治疗的方法对输卵管积水栓塞处理对体外授精-胚胎移植(IVF-ET)结局的影响。方法 2009年11月—2012年6月对单侧或双侧输卵管积水行病变侧输卵管栓塞术,再行IVF-ET 154例为介入治疗组,同期单侧或双侧输卵管积水未处理直接行IVF-ET 84例为对照组。结果对照组的足月产妊娠率明显比介入治疗组低(P<0.05),异位妊娠率却高于介入治疗组(P<0.05)。结论输卵管积水降低了IVF-ET的足月产妊娠率,并且增加了异位妊娠的发生。而IVFET术前行输卵管栓塞术不影响卵巢功能,同时可提高妊娠率,减少不良的妊娠结局。

Objective To explore the effect of interventional embolization treatment for hydrosalpinx on the outcome of in vitro fertilization and embryo transfer （IVF-ET）. Methods During the period from November 2009 to June 2012 at authors’ hospital, a total of 154 patients （intervention group） with unilateral or bilateral hydrosalpinx were treated with unilateral or bilateral fallopian tube embolization, which was followed by IVF- ET. During the same period, 84 patients （control group） with unilateral or bilateral hydrosalpinx directly received IVF-ET without receiving any treatment for hydrosalpinx. The results were analyzed. Results The full-term delivery rate of the control group was significantly lower than that of the intervention group （P ＆lt; 0.05）, while the ectopic pregnancy rate of the control group was strikingly higher than that of the intervention group （P ＆lt; 0.05）. Conclusion Hydrosalpinx can decrease the full-term pregnancy rate of IVF-ET, and increase the incidence of ectopic pregnancy. The interventional embolization treatment of the hydrosalpinx before IVF-ET does not affect the ovarian function and meanwhile it can improve the pregnancy rate and reduce the adverse pregnancy outcome.（J Intervent Radiol, 2014, 23： 311-313）.
