重症急性胰腺炎合并腹腔间隔室综合征23例诊治分析 被引量：5

摘要 目的 探讨重症急性胰腺炎合并腹腔间隔室综合征患者的诊治.方法 回顾性分析本院2007年4月至2013年5月23例重症急性胰腺炎合并腹腔间隔室综合征患者,通过观察腹腔压力与腹腔灌注压、尿量、血乳酸浓度、C反应蛋白水平、急性生理和慢性健康状态评分系统Ⅱ（APACHEⅡ）评分相互变化关系,探讨腹腔内压变化与患者临床理化指标及预后的关系.结果 患者治疗前后腹腔压力（IAP）与腹腔灌注压（APP）、尿量（UR）呈负相关,与BLA、CRP、APACHEⅡ评分、MODS评分呈正相关（P＜0.05）.治愈患者治疗后IAP、APP、UR、BLA、CRP等临床理化指标及APACHEⅡ评分、MODS评分均得到明显改善（P＜0.05）,而死亡患者治疗前后相关临床理化指标及评分改善趋势不明显（P＞0.05）.IAP、Ranson评分、APACHEⅡ评分及MODS评分是影响患者SAP近期死亡的4个危险因素.结论 控制腹腔压力有利于改善急性重症胰腺炎合并腹腔间隔室综合征患者全身炎症情况状态及脏器功能,腹腔内压力、APACHEⅡ评分、MODS评分及Ranson评分是影响患者预后的危险因素.
